The volume of the dose is mostly water. It’s the % of medication that is different. Pegintron is weight based just like the riba. Pegasus is one size fit all.
Yes different sized Peg molecule is very interesting but which is better no one knows, the seem to work about the same, I did both relapsed on Pegasus 24 weeks tx. Svr on pegintron 48 weeks tx. I think the most important thing is that they are both good meds and one may work better depending on the body chemistry of each person.
There seems to be no other way to describe it than an overdose. But no one ever mentions it as such. There must be tons of people experiencing this, maybe many of those who end up discontinuing treatment altogether. ! I think you're right about lowering the dose of Pegasys. Both pegs have similar svr's and I think I read the lower dose of pegintron has a similar svr rate to the other doses. I can always bump up the peg dose a bit if I tolerate the half dose and I will consider that.
1st dose reduction of PegIntron is to 1 mcg/kg/week, 2nd dose reduction (if needed) of PegIntron is
to 0.5 mcg/kg/week.
If platelets go down below 25,000 the standard protocol says to stop all treatment.
There is a way around the platelet issue if the center will do it. That is platelet transfusions and/or Promacta/Eltrombopag a drug that can raise platelets. But these are temporary fixes so they would be need to be done (transfusions) and taken (Promacta) over length of treatment.

This calculation shows that you need at least 800 mg ribavirin per day, divided in one morning and one evening dose of 400 mg each. This gives you a dose of 14 mg ribavarin per kilo, which is a good dose.
Your doctor might even prescribe 1000 mg ribavirin to you, since many times everyone at or below 75 kg gets this dosage. It depends on what brand of ribavirin you are getting.

I question whether a change in interferon would yield better results if the dose of Peg-Intron was a standard dose. You platelet count would be taxed further with a Peg increase so you would need to be monitored weekly for platelet count, prothrombin time, CBCs and viral load. Basically, you're in a tough place and there are no easy answers. If you weren't having the issues that you are having this would be one of those times when I would suggest waiting for newer drugs.

Talk to your Dr. I switched from a gastro to a hepatologist in my 6th week of tx. My hep dr. told me (a little late) that he would have started me on Pegintron due to my weight. At start of tx I weighed 67 kg (147 lbs) and was on 100 mcg PegIntron. I gained weight during tx, and when I hit 75 kg (167 lbs) my PegIntron was increased to 120 mcg at week 40 of 72.
PegIntron is weight based, weigh more - higher dose (between 80 - 150). Pegasys is one size fits all (180). I'm one who has used both and found that Pegasys is less nasty than PegIntron. Others may say the opposite. But, in informal observation over the years slightly more people think that Pegasys is not as bad and PegIntron.
By the way, some less weighty people have chosen Pegasys for the reason that the doc suggests against - they thinking that proportionally stronger is better.

to compare their dosage level and side effect level to ours. one study i looked at said that a dose of 30 micrograms/week of IM interferon beta-1a was well tolerated but a dose of 60 micrograms/week caused severe flulike symptoms and elevated liver enzymes. im assuming that this type is similar to infergen, really strong.
it seems like pegasys and pegintron dosages are somewhat similar.
you are 67 kilos and your dose should be 1000 riba if you taking pegintron. If you are on pegasys I believe the program is different.
Your hgb is not low enough to be concerned, if it goes under 10 then you should be given procrit rather then a dose reduction to keep the level between 10-11 hgb. I believe that the recommended weight based dosage for riba is 12 mg per kilo, at 800 you are at 11.94 per kilo, at 1000 you are at 14.
