1) I have the beginnings of a cold sore on the lip. For Valacyclovir and Valtrex the recommended dosage is 2000mg every 12 hours...total of 4000mg in 24 hours. I have 2000mg of Valtrex and 2000mg Valacyclovir from prior outbreaks. Can I take them together for the some effect? Or do I need to take the 4000mg total dose with the same drug?
2) I am getting cold sores on the lip up to 3 times a year. There is no common catalyst. Each outbreak if not treated early is as severe as all the rest.
I'm on daily 1 gram valacyclovir for suppressive therapy. I can feel a new outbreak coming. Would it help if a take the valacyclovir twice daily for a couple of days to help with the OB?

Speak with your doctor about increasing the dose or perhaps switching to valacyclovir, which gives higher blood levels and sometimes is more effective than acyclovir.
I am also hearing from you that, while taking the valacyclovir, if I were to miss a dose accidentally then I should probably also avoid sex for a week because there might be some rebound in viral shedding.

I'm sorry, I missed the Hashi's dx - my mistake. I don't see anything that indicates you had a thyroid ultra sound? Please don't tell me I missed that too, in spite of going back and reading again...........LOL
It's possible that you have nodules that are leaking hormone independently of the thyroid gland itself. This can swing you from hypo to hyper very quickly.
I believe (though I could be wrong) valacyclovir is the only drug that is approved by the FDA for suppressive therapy, because it's the only drug that has had an FDA registered clinical trial performed for this purpose. However, it is well documented that valtrex and acyclovir perform very similarly. Valtrex is actually converted into acyclovir by the liver, so essentially, the two drugs act the same way in the body to suppress the virus.
I am conscious it us a holiday weekend and see that you are on currently. Wondering if you are able to answer my last question above around spreading of blisters.

after this doctor told forget about herpes and go to dermatologist. I bot Valacyclovir and start to take it.it helped a bit,but not fully.after I drink alcohol it is burning and tingling much stronger. And also shad left leg pain. After alcohol it is very strong and it is moving and burning. But at the morning and when I walk it stops.Ulcers disappeared but I still have rash on the shaft of penis.after 1.5 half months on Valacyclovir. I take now 1000gr morning and 1000 evening.
t doing well and you raised them on your own to see if it would help which is never a good idea? What is the dose that you are taking? What dose did you take today? I could give you some more information then. But more importantly if you are starting to feel sick then you might have taken above what is the reccomended maximum dosage and clearly you are encountering some side effects.
1) There is a school of thought that the standard suppressive dose of valacyclovir (500 mg once daily) might be on the low side, i.e. that double that dose (1 gram) would be more effective. Since the higher dose hasn't been studied for its prevention effect, there is no way to know with certainty, hence a modicum of debate among the experts. Personally, I would take the higher dose and often recommend it to my patients.
the problem is, his symptoms keep disappearring and reoccurring, despite taking a very high amount of Valtrex (1-2 grams twice daily while having symptoms) and even a maintenance dose in between (1 gram daily). This last time, he had prostatitis in addition, which I read can also be a secondary infection with Herpes. He saw his urologist for that and was put on antibiotics which seemed to work. he hasn't been officially diagnosed yet for HSV2 because he never had sores present.

If both the infected and uninfected members of a couple consistently take any of the anti-HSV drugs (acyclovir, valacyclovir, famciclovir), I cannot imagine transmission taking place.
You are wrong about suppressive therapy and blood test results. If someone catches HSV-2, the blood test becomes positive. If it remains negative, no infection occurred.
For suppressive therapy to prevent transmission, I normally rely on valacylcovir 1.0 g once daily.
