For PEP to be effective, it has to be started very early after the exposure. We are talking hours not days. In animal models of infection, effectiveness of PEP really suffers as the delay in initiation increases. Most experts feel that after 3 days there is no use to starting PEP. Additionally, many also now accept that Truvada alone may not be sufficient for PEP and that adding a third drug is better.
After doing some reading, I see that Truvada+Kaletra (or Truvada alone) is a commonly prescribed PeP regimine. I know that Truvada is part of the Stribild combo, but Im not sure why this was the specialists choice? I saw that there is an ongoing clinical trial with regards to Stribild as PeP, which would mean that it is not yet approved for such purposes?
My questions for you are: Will Stribild even be an effective PeP treatment?
Due to very high risk demographic and the area, I was put on truvada for a month, due to open wounds in the mouth, having not successfully found out her status. Her age group is about 25% HIV infected, I should have thought about that before. I'm still friends with her though she is scared to get tested.

I would not have recommended PEP for you but of course, if you want it, I am sure you can get it. Decisions about PEP are a personal decision in which one should consider the likelihood that infection will occur, the likelihood of drug side effects, one's personal anxiety and the cost of the drugs (expensive, typically in the neighborhood of $1000). When I got home and looked up PEP, it says its usually Truavda in combination with a mix of other drugs. So the next day i went to a local clinic and they echoed that Truvada was enough. However, i pressed them to also give me isentress as well and they complied. Is truvada alone ok?
If my paretner was indeed positive and not on treatment, what are my chances of being negative? I took the Truvada 16 hours post exposure and the isentress at 34 post exposure.
If you didn't have sex why would you take PEP? If I didn't have unprotected sex I wouldn't spend 800.00 for a script of Truvada and take the risk of possible side effects. If you told your risk to any doctor around here you would have received a script.
You are receiving care from one of the most knowledgeable places you could have gone- the folks at the SF City clinic are experts in HIV prevention. The doctors who provided you with your initial PEP provided you with more medication than is typically recommended for PEP and the Truvada (which is a combination of two anti-HIV medications) is one of the regimens most widely recommended for PEP.
Looking at your other thread, you were at lower risk than you think. Even without PEP, your risk of catching HIV in this situation was probably no higher than 1 in a couple hundred -- that's the average risk from receptive anal (bottom) with a known infected partner. If your partner's story of low viral load was true, your risk was even lower.

Thanks for that. PEP is effective, but I'm asking about Truvada in particular. Does the timeframe of 42 hours limit Truvada's effectiveness?
Also, can one missed dose combined with a continuation of the doses thereafter really reduce effectiveness to 33%? Is this the consequence of missing one dose?
t taking any chances so continuted to take the truvada in the hope it will do something - I believe that truvada only was the original PEP can anybody verify that for me? At around three weeks I got pain all down the sides of my neck for a couple of days. I went for a run at the gym and it disappeared.
I have several questions about this that health professionals have been unable to give me a good answer to.
