Avatar n tn 1 month later he was not saticfied with the drop in cholestoral so he put me on a fairley new drug called Trilipix. I take 10mg of lisinopril for high blood pressure, 10 mg of crestor, and 145mg of trilipix. After being put on the trilipix I started felling tired and having stomache problems (constipated and cramping). I went back to have my a1c check last week and it was 8.0.

It looks like a total of 3.5 to 8.5 months to complete the process I go to the VA and my doctor is hoping the process goes quickly and be able to start by next April. I am genotype 2a/2c and been putting off treatment for the past 6 years.

1981227 tn?1336328438 Apart from the Rebif side effects. The formulary issues with Rebif and Kaiser are finally catching up and my Dr is forcing me to switch to Extavia. I'm not happy and told her so, but seems I have no choice... :( So I guess I'm just looking for other Extavia takers, esp if you were an Ex-Rebifer.

The results are below by insurance company. o Kaiser NW Harvoni – not covered Sovaldi – tier 2- This drug is available at a middle level co-pay. Most commonly, these are "preferred" (on formulary) brand drugs.
Avatar m tn That means that Medicaid wont coverage for them without a specific written request (there is a form for this as well) with a doctor's letter. There is a formulary meaning medications they generally cover and other medications might only be specifically covered with this process. However if the follow up is done correctly there should not be a problem. Call the Medicaid hotline for more information and also ask where the doctor should call to request necessary forms.
Avatar n tn Here are the meds I am own - Questran - 1 packet for irritable bowl, Prozac for OCD, Allergy Injections, Methyldopa for blood pressure, and Trilipix for Triglycerides (nearly 1000), and Vitamin D. I am only 30!! I am overwieght but work hard to get rid of it with little or no results. They even checked me for Cushings. My TSH is low at 0.3 and I have a Goiter but no known thyroid problem. I have acne. Anyone know what could be causing my heartburn to be so severe?
Avatar f tn Vicoden is one of many pain control meds thats active narcoric ingredient is hydrocodone - usually along with anywhere from 350-1000mg of acetaminophen depending upon which formulary is prescribed. Its not a toy and isnt much good for chronic pain as far as I am concerned........
315318 tn?1353251800 Recently , I was prescribed a fenofibrate (Trilipix 135 mg) along with Crestor. My cardio wanted this for helping in getting my triglycerides down. I have not had a lipid profile done since starting on this med two months ago. However, since being a diabetic , I check my blood glucose (BG) many times during the day and what I am noticing is a clear trend in the BG numbers going lower at least over the last month or so.
Avatar f tn butalbital is a barbiturate that is in the fiorcet formulary. There is a plain old butalbital formulary. And, similar to tylenol, a combination that contains various amounts of codeine in the pills. You want to stop taking the butalbital very slowly and over a decent period of time. Do not cold turkey a barbiturate medicine.

If you have insurance your best bet is to contact them or through your doctor. If it is covered your doctor will have to proscribe it first and submit for pre approval.in most situations.
Avatar f tn FYI For those on Medicare, Medicaid, etc - Does it have to be added to the "Formulary" of your insurance company?. Can anyone on Medicare, Medicaid, etc confirm coverage now? Who on Medicare, Medicaid, etc will get this expensive treatment? Will there be restrictions that only those who meet certain medical criteria will get it in the near term and others will have to wait? Will it cover off-label.use? The above is required before .panfoundation.org coverage.

Avatar n tn NDC# 51167-100-01 or 02 (blister pack of by the bottle-168) Depending on you RX provider, you need to find out if it is in their formulary so the insurance can process it and find out your co-pay. Im on blue cross and they had it within a week on their formulary. Drug store has to order it. Riteaide said it retails for about $9200 a month (ouch!)...luckily my co-pay is only $70 a month.
Avatar m tn I agree. The benefits outweigh the few, (if any at all) side effects from taking the benzo's every day for over six years. I'm 26 now without insurance, and they are the cheapest of the meds I am on. Trilipix is 184USD, Crestor is 189USD, and even generic gabapentin is 154USD. Lexapro is about 140UDS. Generic Klonopin is about 10USD. Remeron is about the same. And Inderal is 4USD. This is all per month, and I need every one of them.
Avatar m tn twice a day, Amlodipine Bensylate 5mg. once a day, Trilipix 135mg. once a day and Ibuprofen 600mg. when needed. Pravastatin Sodium 40mg. once a day and Alprazolam 1mg. twice a day. I have been to my doctor and have told him about it and he just says, its anxiety. Does anyone know what might be causing this ?? I am tired of hearing that it is anxiety ! Please help !!!!!!!!!!!

This has been an ongoing discussion for several months. Have you talked with your insurance company about the drugs and about coverage for the drugs? That would be the first place to start. I know that before I started treatment with Interferon, Ribavirin, and Incivek in Sept.

We should probably put together another log type comment to ask people how soon it showed up on their formulary, how they're paying for it, what country or part of USA they're from. All that interesting stuff.

Drugs in each tier have different costs. So not only is being covered important, but what your co-payment will be. So prices may vary between plans that both cover Sovaldi. A drug in a lower tier such as Tier 1 will cost you less than a drug in a higher tier.
Avatar n tn I am now on medicare and anthem blue cross will only provide me with a 30 day supply. The reason given is that oxymorphone HCL ER 7.5 MG TAB is not on their formulary. Without the insurance this medication cost $300.00. I take it 2 x a day and it is very addictive. I did think that we/they understood that using blue cross was an option only if this drug was covered. What to do?.... In continuing, it seems I've come to the wrong site.

Avatar m tn Trimix as you may have realized is a Rx Formulary which has several variations on the mix of three ingredients which the Urologist prescribes. The prescription is in vial of usually 10 CC and is injected in the side of the penis shaft. Syringe is usually 1/2" 30 gauge,with initial dose of .15cc. A powder form/fluid and mixed within inhector, is available yet not commonly prescribed and that is injected into the uretha.
Avatar f tn Actually, insurance should not deny, in my opinion, unless you are taking a drug that does the same thing or it is not a covered drug under their formulary. (You would check with your insurance to see if Lexapro is on their formulary, because those books they mail out don't always list everything).
