Myocardial ischemia does not commonly occur at your age. But with the high risk factors and if there is a strong family history it can occur. In the Framingham Heart Study, the incidence of an MI over a 10-year follow-up was 12.9/1000 in men 30 to 34 years old and 5.2/1000 in women 35 to 44 years old. In two series of patients with CHD at ≤40 years of age, women comprised 5.6 and 11.4 percent of patients.

im 25 years old and a smoker for many years. there is no heart disease that runs in my family that im aware of. but at my age can i still have a heart attack...lately i get constant pain up and down my arms and into my shoulders then center of my chest hurts and it feels heavy but not real bad it kinda feels like heart burn. the pain in my arms will only be in one arm at a time. the past 2 days the pain in arm and chest comes and goes..

As someone who was born with a bicuspid aortic valve (BAV), I can tell you that the high blood pressure, the murmur, and the emergence of symptoms in adolescence are consistent with BAV. I'm not saying that BAV is what your son has. I'm saying that it's something to rule out.
