Being that you have been taking hydrocodone/acetaminophen for 5 years it is probably best to switch to a long-acting opioid and use the hydrocodone/acetaminophen or a similar short-acting opioid for breakthrough pain.
I have been taking combination short-acting opioids for over 4 years now and I was just recently switched from percocet to oxycodone IR for breakthrough pain.
​Avinza 90 mg and 120 mg capsules are only for patients in whom tolerance to an opioid of comparable potency is established. Patients considered opioid-tolerant are those taking at least 60 mg of morphine daily, at least 30 mg of oral oxycodone daily, at least 8 mg of oral hydromorphone daily, or an equianalgesic dose of another opioid for a week or longer.

AVINZA capsules are a modified-release formulation of morphine sulfate indicated for once daily administration for the relief of moderate to severe pain requiring continuous, around-the-clock opioid therapy for an extended period of time. My third visit to PM Dr is in two weeks and I am not getting any relief. I take my medicine as prescribed. I went from 10/325 oxycodone 4x a day to 30mg avinza, and now I am taking 45mg avinza still with no relief. My pain remains between 5-8 with 9 most evenings. I dont want to be labeled by telling my doc I am not getting any relief.
I am looking for suggestions.
I have been on prescribed oxycontin for 3 years. Before that it was prescribed methadone and loratab. I also have prscribed morphine. I have been taking half for two days and taking whatever is necessary to sleep. My question is can i be withdrawing from the oc's while taking morphine?

I have been in pain managment for 4 year with back and neck issues. I am prescribed Oxycontin 40mg 4x's daily and hydrocodone 10/325 4x daily. Thinking of switching to the patch. Do I need to wean off oxy before using the patch. Do i need to worry about od ing because of the halflife of oxy? When I convert your current dose of MS Contin into Oxycontin equivalent, you are taking 60 mg less of Oxycontin through the MS Contin. To get dosing equivalent to 120 mg of Oxycontin through administration of MS Contin, you should have been prescribed 180 mg of MS Contin based purely on the pharmacological conversion factor between Oxycontin and MS Contin as Oxycontin is 1.5 times stronger than MS Contin.
Why any doctor or pain management clinics would switch to oxycontin is beyond me. MS Contin is bad enough, Oxycontin is far worse. I was addicted to oxy within a day and spent most of my time sleeping about nineteen hours a day. I was too lethargic to get out of bed other than going to the bathroom or grabbing a bite of food which had no taste and which I ate very little of. The latter list of medications are known as adjuvant medications which help the opioid medications work more effectively.
The most effective way to control chronic pain is by prescribing a long-acting opioid medication to cover your baseline pain level and also by prescribing a short-acting medication for breakthrough pain which is pain that occurs in between dosing of the long-acting medication.
My PM doctor's nurse put me on various drugs until Avinza, then I stayed on it for the last few years. I have severe, life threatening Respiratory problems and my memory is getting much worse. I am 100% disabled from a car wreck. I am medically retired. My wife is here with me, so I am not alone. I got my dosage down from a high of approximately 450-500 mg a day to 30mg of Avinza a day. Oxycodone is for BT pain. I took my last Avinza at 10pm Sat, July 19.
I have been on opioid pain med since march when they stopped the cortisone treatment for my shoulder (stopped working) I am going in for total joint replacement tomorrow. The hospital staff said I neede to stop all blood thinnig drugs (aspirin, fish oil, advil, etc) but did not say anything about pain mends (Oxycontin 4mg 2x day and Norco for the breakthrough pain (10 - 50 mg per day depending) Can I take the pain meds right up to the surgery?
i take oxycodone, and oxycontin. I build tolerances very quickly, but i never really experience any "high". The only way i know something is working if for the pain to go away. I have been reading on fentanyl and opana. I'm in pain all the time but don't want to have to take a thousand pills a day. I don't mind having different types around depending on my pain level, but i just really want something that works and I can be coherent while taking. Any advice?
My daughter was told at her pain clinic that she could not build a tolerance to Avinza. My daughter has to be taken off of Avinza and put on something else for a while and go through terrible withdrawl, including restless leg syndrome, then she sees the nurse again and has to argue with her to get her to put her on Avinza again.
anyone have any problems with Avinza? (morphine sulfate extended release). My dr said that it had an antagonist that made it non addictive. He prescribed it to me alone with Norco (high dosage)- I stopped taking both, and it's been 6 weeks, but I'm still struggling.
Would substituting hydrocodone for oxycontin help me in my efforts to withdraw from 40 mg twice a day of oxycontin. I'm getting terrible leg pains and restless legs, plus stomach bloating, cramps and severe digestive problems. I thought something was really wrong with my stomach until I read that those are normal detox symptoms. Any suggestions would help as my doctor is useless and don't want to take me off the meds but yet he claims I'm addicted to it. What can I do.
