Common Questions and Answers about Add questionnaire

Avatar f tn I'm still a work in progress :o), so I can't help with your questionairre; however there is a recent article in Clinical Care Options that discusses the term "cure" as is used in HCV management. Here's the addy: You'll probably be asked to register, but it's quick and free.
Avatar f tn I believe you can go to ADD/ and it has a questionnaire as to whether it has gone from childhood to adulthood and is causing any problems. Read about it and see what you think. Goodluck!
Avatar f tn It varies to a degree and in order to be prescribed stimulant medication you just complain of those symptoms and possibly take a questionnaire to prove such claim. ADD/ADHD is just a name. It used to be called minimal brain dysfunction. It's a name for doctors, it's a name for lawyers, it's a name for the pharmaceutical business people.
Avatar f tn I think it could very easily not catch a child with ADD. The gold standard for diagnosis is to do a questionnaire with the teachers and parents. ADD and ADHD are very different. Good link here - My personal opinion is that this test is just a way for inexperienced professionals to deal with a problem they have little knowledge of. Are you in the United States (which state) or England?
602796 tn?1219877065 Why is it that we are quick to diagnose children with ADD or ADHD?? A child or teen is unable to pay attention in class, and they suddenly have ADD or ADHD! This is ridiculous! Children in the first place, are always wanting to learn. And the fact that they are unable to sit still, could just mean they are bored. People just need to understand how to speak their language!! Their brain activity is moving at such higher speeds than the typical person next to them!
Avatar f tn To add, I've also had some of these symptoms from hypothyroidism such as internal tremors, neck pain and lightheadedness. Some people also suffer migranes, night sweats, insomnia from hypothyroidism as well.
Avatar f tn forgot to add - my white cell count was 10 crp was 3 history, hysterectomy 12 years ag0 left ovary and cyst removed march this year but pain is always right sided, l am really really scared as pain radiates in case of cancer, please help me put my mind at rest thank you
Avatar m tn Hello and hope you are doing well. Your symptoms of seeing things could be due to sleep disorders “Nightmare” or "Night terror" disorders . Here the person experiences symptoms of bad, frightening dreams which interfere with his sleep schedules. Disorders related to dreaming could occur with REM sleep behavior disorders. Here the person acts out the dreams. This can be assessed by a sleep questionnaire and therapy is based on the severity of symptoms.
Avatar m tn There is a questionnaire I have heard that ment had to have their doctors fill out about their medical condition I guess my doctor did too but for me it was all behind the scenes I call my drug insurance provider and bugged a few people some faxes were exchanged and voila I had the meds. My advise would be not to give up and find out more about the denial. Yes both drugs are FDA approved since Nov 13 and Dec 13. To use them "off label" is the issue.
12030399 tn?1423228044 My husband just sent me a text saying he called them and they are sending me a questionnaire to see if I qualify for the treatment? I'm always really iffy about things like this and just wanted to see if anyone has heard of him.
484180 tn?1209160023 When she was 8 a teacher casually mentioned to me that she may be ADD. I was uneducated about ADD at the time, and just thought although very intelligent, she was a daydreamer. Throughout the years, she has struggled at school and at home with difficult and simple tasks. She is now being evaluated by her doctor and at school. The two teachers at school, to my suprise, dont feel the same way I do.
627816 tn?1349241716 pdf Diagnostic Checklist Form E-2 and Research Questionnaire Form E-3 A high-priority goal of the Autism Research Institute (ARI) is the development of improved methods of diagnosing children with severe behavior disorders, since most medical research is highly dependent upon accurate diagnostic methods. Our Diagnostic Checklist, Form E-2, has been designed for this purpose.
Avatar f tn Will do! You are now officially "counted"!
Avatar m tn Any chance he has something like ADHD or ADD? Here are two links that should help you decide if he might. I am surprised the teachers have not asked for testing of him. You might also want to look into buying "Cool down and work through anger" or "When I feel angry".
Avatar f tn How to balance your brain chemistry using nutrition, exercise and lifestyle I have made available for my clients the detailed questionnaire which clinicians have used to determine neurotransmitter dominance and deficiencies. I took the questionnaire recently and found that the questionnaire accurately identified my neurotransmitter deficiency which was corroborated by testing done by an associate of mine who is a naturopathic physician.
Avatar n tn I also wanted to add that he has some sensory issues as well(not diagnosed) he cries or screams when he hears loud noises like a hair dryer or a blender and often times a song of music on tv scares him. I have a hard time getting him to eat as well. He wont even try a food most of the time even if its something I know he likes. I know hes hungry though because he will eat cereal all day long if I let him.
Avatar m tn You may need a Functional Medicine or Naturopathic Doctor to help you diagnose and treat you. I have an Adrenal Fatigue Assessment/Questionnaire and if interested, I can send it to you in a PM. Your profile says male, but are you? Some elements of the AF investigation are gender sensitive.
255722 tn?1452550141 My experience with life insurance "problems" started about 30 years ago. Like you, I had some "heart tests" and all of a sudden, I am considered a risk! I was diagnosed with mitral valve prolapse (a relatively benign condition) and pvcs. The doctor was not at all concerned, just a blip on the chart. WELL, the insurance company felt differently. I was about 20 years old (eek!) and I was given "smoker's rates" because of a "heart condition.
1558393 tn?1299073631 Hi, I was wondering if anyone has sent the questionnaire, MRI's, and your operation report to them for their imput? I have filled mine out and sending it off this week with a copy of my insurance card. I have to do something the headaches and body pain are getting so bad. Can't find a doctor here at home that knows enough about it to help me. I was wanting to see if anyone sent theirs and has got a response from them and how long it takes.
Avatar m tn Below I have included an Adrenal Questionnaire which may be indicative of adrenal fatigue, more like a preliminary test. Adrenal Questionnaire Read each question carefully, and record the number next to a question if it applies to you. When you finish, add up the numbers you have recorded. Do you experience fatigue? 3 Do you have allergies? 3 Do you have asthma? 3 Do you have recurrent infections? 3 Are you under severe emotional stress?
Avatar f tn All it requires is completion of a questionnaire (ok, a really loooong questionnaire) and giving up several vials of blood. There are no repository sites in Canada but I was able to participate a few years back when I was visiting NYC. Also, FYI for Canadians, if you contribute financially, you arent eligible for a tax receipt, but hopefully that does not stop you from donating. Thanks for posting Ess, this is great info for our new members.
Avatar f tn It seems like not only is your doc not listening to your concerns and trying to make any changes that would help, but it sounds like you may be misdiagnosed as well. Treatment for ADD versus an anxiety disorder will vary greatly. If you DO have ADD, treating you for anxiety could actually be making all of your symptoms worse. Please seek out an assessment from another doctor and explain to him/her what you've been through and what you've tried thus far. Best of luck to you...
Avatar f tn ADHD/ADD is a physical issue and I feel you should be seeing your family doctor. The covering of his ears because the sound is too loud is also indicative of his ADD/ADHD. I am wondering if the behavioural issues are a result of the poor management of the ADHD. As always, start with the family doctor .....
332747 tn?1198123717 Just wondering how many of you out there are ttc using ds and are in their 40's and single (sounds like a dating add l.o.l)? I would like to share our stories. 372366 tn?1284407473 test results what do you think by HARRYBEADS Send Message Add as Friend View Profile HARRYBEADS ON Mood: HARRYBEADS is missin his bike , Mar 13, 2008 11:07PM I would like to know what the Doc thinks of these results of my neurological test.I will not be seeing the Doctor for a month or so and I'm loosing it with worry.
Avatar f tn I'm really concerned for him because he claims to be afraid of sleeping at night and some nights he refuses to sleep because he doesn't want to risk not waking up in the morning. I know he's on an anti-depressant and ADD medications. Could those be bringing this on? I don't know when he was diagnosed in relative to when he started taking those medications, so I'm just throwing this out there.
Avatar m tn The test indicated that there is a mild to medium intellectual deficiency (synonomous with mental retardation, although I did not know this interchangeable use of terminology by MDs until I did my research). The conners questionnaire filled out by myself and the teacher indicates ADHD in addition to learning disabilities. I would like to know if ADHD can lower IQ tet scores due to the fact that he could not concentrate on the actual test.
3227684 tn?1347376665 It is not terrible, because I am always able to wake up with efforts, but I think these symptoms were less significant before. I must add that this fatigue arises only in the morning. I am sometimes exhausted after a long day, like everyone, but overall, I am in a good shape. I must say that I go to bed quite late (generally around 01:00 a.m.), although I wake up late too (around 10:00 a.m.).
1936411 tn?1333835449 ) Pages 11-13 feature the McGill Pain Questionnaire, which may be useful to some of us, and ties to recent forum discussions of how to describe symptoms. I hope this is of some value to members.
408795 tn?1324939275 That is an excellent question. I wanted to add more life insurance to my existing policy but had 2 things that made me think it wasn't going to happen 1) a long questionnaire that must be signed by your medical team explaining your current illness and 2) I am 35 years older than when I bought the original life insurance I just let it drop because it would cost so much and take a great deal of my time to get approved.
