I was diagnosed with Depression with psychotic features, not any Bipolar disorder Anyway I was on Prozac with Wellbutrin, then had a relapse and they took me off both and started me on Abilfy only. Is this a normal procedure or should I also be on some low dose antidepressant in your opinion ? Thank you very much for your time.

As others here have commented, a real psychotic episode, which is a very serious event, must occur in order for their to be a true 'bipolar I' diagnosis. The diagnostic 'bar' for labling someone as 'manic depressive' must be high, because of the risks of the medications that are needed to control this illness, among other factors.

Aspects of psychosis may also manifest during extreme episodes of depression. They are also present in schizophrenia and schizoaffective disorder. These features include delusions (false ideas about what is taking place or who one is) and hallucinations (seeing or hearing things which aren't there). As you can see those suffering from Bipolar I are more likely to suffer a psychotic episode than someone diagnosised with Bipolar II disorder.

I experience the same type of hallucinations thoguht i think its down to a side effect of meds i can be quite paraniod sometimes as well it sets in on me quite quickly and i find it hard to control sometimes. I think you would mean depression with psychotic features and like most psychiatric disabilities it lasts a lifetime as does the need for medication. Recovery rates depend on the individual person. As I've posted medications that are more specifically targeted to the right receptors and will have a better response rate and safer side effect profile are in development so treatment will improve in the coming years.

It sounds like he possibly has severe depression. Some people with severe depression also have psychotic features (depression with psychotic features). s not generally a part of depression but it can be from depression with psychotic features or other related conditions but only a psychiatrist would understand the specifics of it but ask them where the paranoia comes from. Recovery rates from antipsychotics vary but they will improve as new treatments become FDA approved (you can read through my posts for more information) so don't give up hope. But do discuss the whole issue with her psychiatrist.

I know of four bipolar 1 with or without psychotic features, bipolar 2, bipolar nos, and cyclothimia. With bipolar 1 you get full blown mania and major depression, with bipolar 2 you get hypomania and major depression. cyclothimia fluctuates between minor depression and hypomania. Bipolar nos is where they put you when you obviously show some symptoms of bipolar disorder but they do not know which classification to put you in.

Bipolar has no aspects of psychosis. With bipolar with psychotic features, during a period of moodswings (mania, depression) a person has auditory hallucinations (hears voices) or visual hallucations or has delusions (perceives things that are not in reality). With schizoaffective disorder, a person can have hallucinations or delusions even when the mood is euthymic (normal).

There are so many different varities of bipolar as well from cyclothymia (mild bipolar) to bipolar with psychotic features to schizoaffective which is schizophrenia with aspects of bipolar (what I have and have recovered from). Depakoate is a mood stabilizer. Zoloft is an anti-depressent. Seroquel is an antipsychotic and I believe by psychiatric standards that is a subclinical dose (when I was on it 300 mg. was where it started working). Not sure what "the pill to calm her nerves" is? You mean an anti-anxiety medication? Although only a psychiatrist can determine medication dosages and which medication to use I'd suggest you inform yourself.

In mood swings there are difficulties in the understanding of emotions and relations with people and ability to focus and understand things and make appropriate decisions but when a person encounters psychotic symptoms there is a full break in reality testing.
