As others here have commented, a real psychotic episode, which is a very serious event, must occur in order for their to be a true 'bipolar I' diagnosis. The diagnostic 'bar' for labling someone as 'manic depressive' must be high, because of the risks of the medications that are needed to control this illness, among other factors.

He was diagnosed with Major Depression with psychotic features. He has been on 20mg of Zyprexa, 100mg of Wellbutrin, 60 mg of Prozac for three years. Is this a normal course of treatment?

When I was younger before my diagnosis I had similar psychotic features and I was treated for major depression with psychotic features. I was treated with anti depressants and anti pyschotics. Unfortunately the anti d's triggered rapid cycling and lithium was then added to the mix. If it helps I no longer hear voices, it stopped around age 20. With severe depression you can have psychotic features. Also, with bpd, when you get extremely stressed you may experience transient psychotic stuff too. This is short-lived and you should be OK at reality testing (which makes it different from other psychotic disorders). You could try looking the diagnoses up in the dsm-iv. The dellusion can not be cured but the occurence sure as heck can! I am Bi Polar with psychotic features and was experiencecing someithing very similar to what your son goes through but it was more a rage/angel and demons thing - SEROQUEL! Total life saver. With bipolar 1 you get full blown mania and major depression, with bipolar 2 you get hypomania and major depression. cyclothimia fluctuates between minor depression and hypomania. i hallucinate and im often aware of it and i know ive been pretty paranoid and delusional for much of my life. Bipolar has no aspects of psychosis. With bipolar with psychotic features, during a period of moodswings (mania, depression) a person has auditory hallucinations (hears voices) or visual hallucations or has delusions (perceives things that are not in reality). With schizoaffective disorder, a person can have hallucinations or delusions even when the mood is euthymic (normal).

Melancholic depression is honestly just the same as Major Uni-polar depression. Just a different twist on the name. The treatment options are the same as with Uni-polar depression. s not generally a part of depression but it can be from depression with psychotic features or other related conditions but only a psychiatrist would understand the specifics of it but ask them where the paranoia comes from. Recovery rates from antipsychotics vary but they will improve as new treatments become FDA approved (you can read through my posts for more information) so don't give up hope. But do discuss the whole issue with her psychiatrist.
