Pizza can be tough to estimate, more because the carb content can vary so much from slice to slice and pizza to pizza. But although I'm just a mom like you, and not a doctor, it sounds like it's not just pizza causing high blood sugar.
You didn't say what sort of long-lasting insulin she's on-- is it Novalin? Or Lantus/glargine? If you haven't tried Lantus (aka glargine) yet, I would strongly recommend talking to your endo about it.
Montefiori Medical Center (Bronx, New York) performed an in-depth study of 240 women receiveing Lantus (insulin glargine) during pregnancy. The results were published in the Journal of Reproductive Medicine, April, 2009 (4) 208-10. They found the drug to be well-tolerated and advised continuing the use of Lantus during pregnancy. PubMed has no studies whatsoever that substantiate an adverse affect on pregnancy.

Lantus (insulin Glargine) is a very SLOW release insulin, a so-called "24-hour" insulin. Because it does not rapidly alter glucose levels it is safe to use when driving. It isn't adviseable to increase the dose of Metformin. Other insulins have the tendency to rapidly (comparatively) alter levels of conscious and situational awareness. It is often prescribed together with Metformin and Glipozide. I would also hazard a guess that weight control is an issue.

We also need insulin to cover the carbohydrates we eat at meals & snacks. Type IIs sometimes use a combination of insulin and oral meds; some Type IIs use just insulin; others use just oral meds; still others can gain control with diet & exercise.
It is a once a week injection that I take along with my daily Metformin and nightly Lantus injection. It has worked really well over the past year. Has anyone else used it? Others I have spoken with have never heard of it. And on top of that, I spoke with a customer service rep at Brystol-Myers who said it was not intended to be used with insulin.
The other comment by imediject is another possibility. If you are injecting Lantus as your long-lasting insulin, and are injecting it in the morning, the Lantus will probably wear off between 20-24 hours after injection, causing glucose levels to rise in the morning until that morning's dose can kick in and cover it. You need insulin whether you eat or not, for a normal body produces and uses small amounts of insulin all day long, not just when we eat.
if you get a bump after an insulin injection it means that you chose a poor injection site. Insulin absorbs into subcutaneous fat. So wherever you shot and a bump comes up, that means there was not enough fat in that spot to absorb the insulin you just shot.
try picking a more fatty area and shooting very slowly.
I have read on other sites of people having similar unexplained problems with lantus, longtime T1s that describe the feeling as being like a big spurt of insulin from the pancreas but they are long past a possible honeymoon phase. All conclude it's haywire absorbtion. I take one bedtime dose, do not mix, and always use a new syringe. Does anyone have experience with this or know of more detailed info on Lantus absorbtion and stability? Thanks.
