Avatar n tn ECG last month showed SVES, RBBB. This week he has stress test with dipyridamole protocol, and injection of thallium-201. The result showed "evidence of mild, small to medium size, irreversible perfusion defect in the inferior/inferolateral wall (mid to basal level)." Could you explain what this means? Does he need to have surgery or pacemaker? thanks in advance for your help.

Avatar m tn Hello: In my opinion I don't think it is appropriate to evaluate or base risk of these radionuclide based on data extrapolated from external sources of radiation. And l really don't think the risk from a single Thallium scan to be minimal; instead it is very significant. The risk from this radionuclide comes in two forms: one the radiation they emit and second by the process they decay (i.e. Auger decay is highly mutagenic). This is specially true in the case of Thallium 201.

Your Doctor has an interest in cardiology but I can't understand why he has chosen a nuclear perfusion scan at this stage? I had two blocked arteries and a nuclear scan said I was absolutely fine. My Cardiologist said "it's only really good for one thing, to see if there are any areas of dead muscle, apart from that, it's next to useless".
Avatar n tn I've had two nuclear stress tests in the last four years. The first one was done with technetium 99 which i've read gives you 500 msv of radiation and has a half life of 6 hours, but the second one was done with 3.0mci of thallium 201 and 22mci of myoview after stress.

Avatar m tn A classic example could be a patient with strong Angina who has a Thallium scan, which shows good results. This means (in most cases) collateral vessels have opened to feed past the blockage. An Angiogram will not see tiny vessels. So the Thallium shows although there's angina, it isn't really life threatening. What it does say is that there isn't enough blood flow through those collaterals to prevent angina, but certainly enough to keep the muscle alive.

The only possible conflicts could be if injecting you with the thallium isotope was a problem, or your overall health made doing stressful physical exercise risky. But if you don't have any of those problems you should be good to go. It's a workout, but have fun!!
Avatar f tn I had a stress thallium done and got the report today. It shows 2mm ST depression inferolateral leads-abnormal. Myocardial profusion scan normal, no wall motion abnormalities. I am a 37 yr old female, overweight with a strong family history of early heart disease. It was a chemical stress thallium and they wouldn't let me walk on the treadmill instead.
Avatar f tn I recently had a Thallium stress test which came back positive. The sets of images were taken by two different techs while I sat upright in a chair. Not wanting to believe the worst, I went to another Cardiologist. There I had the Cardiolite test, images taken by the same tech while I lay on a table. Those results came back negative. So who is right? Would having a calcium-score CT and perhaps a carotid ultrasound be the tie-breaker? I am 55 years old, post menopausal.

I went to see my cardiologist after the thallium scan, thinking I was going to be told half my heart was dead or something, but I was shocked, as was the cardiologist, to learn none was dead. The cardiologist said that the thallium revealed excellent blood supply to all areas of the heart, and yet I was still suffering bad angina on any exertion.
Avatar n tn The pictures from the thallium scan showed nothing. He is scheduled for an angiogram and we wonder if this is truly necessary. The only other anomaly he has is that his heart rate is consistently low, getting into the low 50's at rest. He takes no medicine of any kind and does not drink. What could cause the difference between low heart rate, abnormal EKG and the thallium pictures other than a blockage, that is somehow not seen on the scan? thanks for any advice.

If unable to do this, your heart rate will be increased using medication. Stress is a strange word really I think, it simply refers to your heart working harder, at the optimum for your age etc. Hardly stress in the sense of the word is it. Anyway, you have one scan with the heart having been stressed and they take 20-30 mins lying still.

Findings are: Peak stress images revealed hypoperfused infero-septal and apical segment, which showed reversal at rest. Impression: Findings are consistent with infero-septal and apical reversible ischemia. Thanks all!
