He was diagnosed with Major Depression with psychotic features. He has been on 20mg of Zyprexa, 100mg of Wellbutrin, 60 mg of Prozac for three years. Is this a normal course of treatment?

Melancholic depression is honestly just the same as Major Uni-polar depression. Just a different twist on the name. The treatment options are the same as with Uni-polar depression. If I recall correctly, Melancholic depression is just like major Depression but a bit less cronic and severe. It's just an acronim that Psychiatrist use to target the actual severity of the condition. Just pretty much the same as Major depression.

As others here have commented, a real psychotic episode, which is a very serious event, must occur in order for their to be a true 'bipolar I' diagnosis. The diagnostic 'bar' for labling someone as 'manic depressive' must be high, because of the risks of the medications that are needed to control this illness, among other factors.

With bipolar 1 you get full blown mania and major depression, with bipolar 2 you get hypomania and major depression. cyclothimia fluctuates between minor depression and hypomania. Bipolar nos is where they put you when you obviously show some symptoms of bipolar disorder but they do not know which classification to put you in. There is also schizoaffective disorder which can have some symptoms simmilar to bipolar disorder.

I am currently recovering from a severe depression with suicide ideation in which i apparently had psychotic symptoms. Has anyone else had psychosis without knowing what it was until speaking to your therapist about what was going on? and does anyone have any advice as to what to look for in oneself when psychosis starts?

When I was younger before my diagnosis I had similar psychotic features and I was treated for major depression with psychotic features. I was treated with anti depressants and anti pyschotics. Unfortunately the anti d's triggered rapid cycling and lithium was then added to the mix. If it helps I no longer hear voices, it stopped around age 20. Now during an episode I have classic bipolar features.

PTSD involves depression. PTSD is a diagnosis, as is depression. Those with PTSD also have depression. I would say "No"...PSTD is NOT a symptom of depression. psychotic depression can encompass a wide variety of symptoms. i treat people with "psychotic depressions" a lot. Depending on what symptoms people are experiencing, different medications will be used. People will often respond very quickly to antipsychotic meds (even within days). The deeper depressive symptoms may take longer to fully resolve but that does not mean you cannot get feeling much much better relatively quickly.

I am 34 years old and always knew I had depression but did not realize until 2007, that I was in fact Bipolar with Major depression and anxiety? I have been on so many different medications and yet I have not achieved the results I was hoping for or expecting. I knew I would not be cured but thought that my life would improve and it has a little thanks to my meds, especially xanex! I take it for psychosis as it is an antipsychotic and also an antidepressant. It has worked better than Geodon for my negative symptoms but risperdal seemed like it was going to work even better for this but unfortunately I could not handle it. Geodon treated my psychotic symptoms better however. It doesn't work as good as I'd like it to but it's better than nothing. I could not tell you how it works for depression as I am not depressed.

I do “suffer” from long-term depression, but I have never experienced psychotic symptoms which can sometimes happen with a major depression.

This month, I have to go back to the pdoc for a meds check and I think he will be putting me on another dosage of some sorts to deal with my depression. He is adamant that the Lamictal will level me out - but the depression gets rough.

Jaquta - I'm not sure what I'll do if I don't like the results from the psych consult. I suppose I could just continue treatment with my primary physician, as I think he could manage my complaints. I have looked up the dsm-iv for bipolar disorder, and it definitely doesn't match my complaints.

i have been living with this for thirty years, originally misdxd with clinical depression (major depression and dysphoria) it has been only a few yers since new pdoc realised that bp was what i have been dealing with all of these years. i have brought up three wonderful children on my own and it is only now i can acknowledge this fact. my ex husband maintains llittle contact with them. his loss. i too prefer to say at home. any time i absolutely have to leave it is a major challenge.

Bipolar with psychotic features is not that rare. When a person has bipolar with psychotic features they have psychosis specifically during moodswings. When a person has schizoaffective as I do they experience psychosis all the time (as I did before my current recovery). The whole spectrum of bipolar to schizophrenia is linked up and they are just learning how.

I am BP1 w frequent psychotic features. I dont know what is what. This whole situation feels like major BP depression but with a twist. Bipolar has no aspects of psychosis. With bipolar with psychotic features, during a period of moodswings (mania, depression) a person has auditory hallucinations (hears voices) or visual hallucations or has delusions (perceives things that are not in reality). However, a spectrum view of bipolar disorder is becoming popular. For example, you can picture a line with unipolar depression (major depression only) at point A, and with full blown bipolar type I at the end of the line, point B. If a person has major depression only, then they will fall close to point A on the line.
