Avatar n tn Im recently diagnosed with A-Fib and am sheduled for an electro-cardioversion next week (after 4 weeks warfarin therapy). The cardiologist wants to start me on Sotalol 3 days before on an outpatient basis. I've read where Sotalol should only be started in a closely monitored hospital setting for 3 days. (WebMD, NIH.gov, others..), and tells me this is common practice. I would appreciate hraing from others on Sotalol as to how their dosage was started..

Avatar m tn heparin is used because it acts immediately. Warfarin might be given, but it takes a day or two to begin working. But heparin is given by injection or instead usually by IV, so it's not practical except when the patient is in the hospital. Warfarin is oral. IV is better because the dose that is delivered to the bloodstream is more sure. Injecting heparin into the body can results in variable dosing, depending on the nature of where the heparin is injected (e.g.

Avatar n tn Unknown. Warfarin metabolism normally occurs through the cytochrome P450 (CYP450) isoenzyme system, with excretion occurring in the urine as inactive metabolites. It is postulated that large doses of acetaminophen may "exhaust" the capability of the CYP450 system to metabolize warfarin. Additionally, DARVOCET-N 100 TABLETS may reduce functional factor VII. Management: Monitor coagulation status and adjust the dosage of COUMADIN 4MG TABLETS (BLUE) accordingly.

This increase in half-life upon multiple dosing is not considered to be clinically significant or to warrant dosage adjustment for chronic use." I've also read that 5x the half life takes you to a level that's deemed insignificant.
