Here the advise from my doctor regarding this topic - continue with the current medication Viread (tenofovir), monitoring laboratory tests every 6 months including creatinine, if the creatinine were to persistently increased by 0.2 or greater we can change to Vemlidy, TAF. Did you have kidney problem before starting Tenofovir? No worries as this will not resolved any problem... And everybody is different. We'll all react to this medication differently.
checking tenofovir trials on hbv only (no hiv coinfection) kidney problems are in less than 1% patients and i haven't found tx discontinuation because of kidney problems.
hbv has no mutations at all under tenofovir tx and i think we can find data from some patients using tenofovir for more than 3 years even if not on trial about this.
entecavir mono has only 1.2% resistance but has several hbv mutations anyway altough only 1.2% has resistance to entecavir.
Tenofovir is known to have a harmful effect on the kidney tubules and boosting agents may raise levels of tenofovir in the kidneys, exacerbating the harmful effects of the drug.
It is generally agreed that TDF is safe and efficacious for HBV with no drug resistance reported so far. In some patients with previous history of kidney problem, TDF may cause a decline in kidney function, this can be handled by dosage adjustment. Consult your doctor. Reduction in bone density has also been reported for long term users.
Whether you have to use TDF forever is a moot point.

tenofovir improves kideys function especially tenofovir+telbivudine in those with kidneys function.
In the case of a significant worsening of kidney function (this happens to just small number of people taking tenofovir) doctors often recommend tenofovir to be discontinued and replaced with ETV (entecavir).
It would be always nice to have a baseline value of those test results, to compare later values so as to be able to recognise potential changes and then act accordingly.

1. A lot of people report kidney and bone issues with Tenofovir Disproxil Fumarate (TDF). Its supposed to be low but Ive seen a lot of members report it.
2. Both etv and tdf have to be processed through renal tubes so both can impact kidneys but most people are ok. The Tenofovir TAF is supposed to be very good.
3. Entecavir in high doses in animal studies was causing cancer but no human cases reported yet.
4. Tenofovir has a stonger resistsnce profile for mutants.
If you are a kidney transplant patient, I presume you are on a lot of medications. How they interact with your HBV treatment is beyond my knowledge.
In simple Hepatitis B patients after several years of anti-viral treatment, hbvdna should be undetectable or at least under 100 iu/ml and ALT should be under30 IU/L. Of course, these are just for "normal" patients.
BTW, Tenofovir has side effect on the kidney, especially for patients with existing kidney problems.
i was checking if there is anyway to prevent kidneys damage in cases like mine where kidenys cannot stand tenofovir, i don t have plans to add tenofovir for the moment but in any case i'd like to make sure i will be able to use it if ever needed.

It is a safe and very potent antiviral but you will want to make sure your kidneys can handle the medication, as it is excreted through the kidneys so can be too hard on those with kidney issues already. Have creatinine checked first.
Another thing, antivirals like Tenofovir or Entecavir are not a medication to take for only a month.

tenofovir (viread) is much stronger than baraclude and has 0 resistance but have some mild sides in 1% cases of kidney toxicity and bone demineralization.seroconvertion on tenofovir is similar to interferon and in case of past use of lam with resistance or adefovir with no resistance is the best safe choice.
baraclude (entecavir) has 1.2% of resistance and on animal models causes cancer at doses 40 times higher than those in humans.
