I am taking Flovent, Spiriva and ProAir. Since I've been on the medication I've been Dxed with idiopathic neuropathy, cervical radiculopathy, carpel tunnel. I also have other entrapment symptoms, ulnar ,and I think in my legs as well.
Sometimes I get a burst of nerve pain in my arms and legs a few minutes after using the Flovent and it also causes some tendon swelling. I've shown over and over that I have less pain on lower doses of meds.
If you are taking Duoneb there is no point in taking Spiriva because Spiriva is related to the ipratropium that is a consitituant of Duoneb. Also Spiriva is extremely drying which may be why you are feeling so uncomfortable.
To be using Duoneb five or six times a day you would have to be close to death. Obviously you aren't, so you can cut back without any problem. I have serious COPD and my use of Duoneb averages 3.5 vials daily.
The big question is - how bad is your COPD?

Because of serious side effects I quit the use of inhaled steroids two years ago. (I am at the lower edge of stage II COPD). I substituted natural anti-inflammatories and my breathing is better for it. What had been serious bronchitis abated. I continued to use DuoNeb and Zyrtec. But the real improvement was when a respiratory therapist suggested I take nebulized Mucomyst (acetylcisteine). I have not coughed one single time since I started using it and my FEV1 has improved 20 per cent.

Mucomyst is back on the market. That will dissolve the phlegm and has a neutral ph, unlike stuff like Spiriva which is acid. (Fungi thrive in an acid environment.) Also you might want to take a potassium iodide supplement.
I had been hospitalized for 3 years in a row all in the same month march, for shortness of breath. They thought it was asthma. The last time which was this year, they hospitalized me for a week. Which they ran lots of test and determined I have COPD. The symptoms I had were shortness of breath, tired, and coughing. Since your panic attacks are happening less, are the headaches numerous and a reason that you cannot take the medication anymore? Or are some of the side effects okay with you now that your anxiety is much better?
What does your prescribing doctor say about this? Headaches can also be caused by an endless number of factors in my opinon. Are you getting enough sleep? Are you still under a lot of stress? The point is, that the headaches may not even be related to the medication itself.
