Yes I would suggest a left heart catheterization to further evaluate you for the presence of severe obstructive atherosclerosis. Apart from this you should be on maximal medical therapy including beta blockers, ace inhibitors, aspirin and plavix as well as a statin. Is you heart function preserved after the heart attack? I'm concerned for pulmonary embolism given chest pain, shortness of breath and HR 126bpm. A pneumothorax can also give you similar symptoms and tachycardia.
