Avatar n tn There is no good evidence that antibiotics work for prophylaxis at all. Also, giving patients unnecessary antibiotics may lead to increased bacterial resistance. These two facts combined led AHA/ACC, with guidance from the infectious disease specialists, to change the guidelines. Of course, we are monitoring eveything that is happening and all trends are recorded. If there is evidence that this approach leads to more cases of endocarditis then I am sure the guidelines will change.
Avatar m tn Now according to revised guidelines by the american heart association for prevention of infective endocarditis, the antibiotic prophylaxis is not routinely recommended for all dental procedures. For patients with underlying cardiac conditions, prophylaxis is reasonable for all dental procedures that involve manipulation of gingival tissue or the periapical region of teeth or perforation of the oral mucosa. So, if the procedure involves the above manipulations, you may need prophylaxis.
Avatar n tn I took medication before all dental visits due to MVP guidelines and then my EP Cardiologist 3 years ago said guidelines changed and my heart murmurs, pvc', pac's were not an issue. The echocardiogram looked fine and I would see him yearly but no need to pre-med. Then I had extensive dental work, bleeding and issue and then my pvc's,pac's increased and have for a couple years now. My fear was that I was not being and continue not to protect myself enough. I respect the M.D.

I had a doctor prescribe a maintenance dose of acyclovir three years ago to my now ex-boyfriend (yes, we were also using condoms, but he and I were both concerned that condoms alone would not protect him). A partner of mine once became infected despite careful condom use, so I am very conscious of the need for better protection. Thanks so much for any information you have!

Regardless, this worries me. He said he has never seen an endocarditis in this situation. How big can the denominator be with MVVs? And for a rare event, would he have seen enough to make the recommendation?

I think my cardiologist told me back in May, shortly before the new guidelines were published on the AHA site. Yipeee!! .
