Vitamin B12 deficiency causes extreme fatigue and either high or low cortisol levels. I had severe adrenal insufficiency due to severe vitamin B12 deficiency. Sounds like you need to start thyroid medication again too. That causes puffiness in the face and eyes and fatigue and stress on the adrenal glands. I have hypothyroidism as well just to add.

Discuss these options with your doctor when you happen to meet him next time. In the meantime you could follow certain precautions. Avoid smoking and alcohol. Low transferrin can be due to poor production of transferrin by the liver, due to liver disease or excessive loss of transferrin through the kidneys into the urine. Also, in liver disease there could be excessive bleeding leading to anemia. What was your hemoglobin level, this will indicate anemia.

High MCH is due to large red cell or macrocytic anemia and occurs due to vitamin B12 and folate deficiency and due to liver disease. So, even though your B12 and folate levels are normal, they may actually be low and being managed by supplementation. The cause behind low vit B 12 needs to be found since you need to take its supplements to maintain normal blood levels.

I don’t have a full description of your consciousness during the lack of speech episodes to decide whether this start is a partial epilepsy which can continue in a generalized or it is a generalized one from the first moment..

Weight gain is not a documented side effect. It could be related to your alcohol consumption or wrong dietary habits and lack of exercise. An occasional one or two drink may be fine but moderate or large amount can cause significant increases in blood levels of phenytoin due to induction of hepatic enzymes.
