Common Questions and Answers about Phenytoin and protein binding

Avatar n tn Such medications as carbamazepine and phenytoin have been shown to displace thyroxine from plasma protein-binding sites and potentially increase its clearance. Free thyroxine (T4) and triiodothyronine (T3) will be increased, while serum T4 and T3 levels will be decreased. Authors have speculated, therefore, that thyroid-stimulating hormone should be followed to determine if a patient on anticonvulsants has normal thyroid levels (JAMA. 1996;275:1495-1498).
Avatar n tn The physician made a statement about skin and lung cells not containing those specific cells and that is the basic point of my question. Again, if I have posted this question in the wrong forum, I apologize, but I saw other questions posted that a doctor responded to and I thought that this was the correct place to post my question. I will certainly look elsewhere - if all that is available here is information from nonqualified individuals like yourself.
Avatar n tn Most of the thyroid hormones in the blood are attached to a protein called thyroid binding globulin (TBG). If there is an excess or deficiency of this protein it alters the T4 or T3 measurement but does not affect the action of the hormone. If a patient appears to have normal thyroid function, but an unexplained high or low T4, or T3, it may be due to an increase or decrease of TBG. Direct measurement of TBG can be done and will explain the abnormal value.
798555 tn?1292787551 attempts to correct for changes in thyroid binding globulin due to liver disease, protein losing states, pregnancy or various drugs*** - guess this is what hes doing.
Avatar n tn The binding protein is known as thyroid binding globulin (TBG). Increased TBG reduces total T4 and T3. Increased TBG levels may be due to hypothyroidism, liver disease, and pregnancy. Decreased TBG elevates total T4 and T3. Decreased TBG levels may be due to hyperthyroidism, renal disease, liver disease, severe systemic illness, Cushing syndrome, medications, and malnutrition. TGB does not affect free T4 and free T3.
Avatar m tn The packaging says it can raise sex hormone binding globulin, thyroid binding globulin, and other binding globulins. I was also menstruating at the time and I think I had a touch of the flu or a cold. My doctor didn't seem too concerned and just wants to re-test my protein and nitrogen December 29. He brushed off my groin area pain as a pulled muscle or something. 2 months away! I think I might go crazy between now and then.

Avatar m tn gov/drugsatfda_docs/label/2013/021356s046,022577s003lbl.pdf] and references 20, 21, 23, and 25). The finding of a 26% increase in vitamin D binding protein and a 42% decrease in free 1,25-OH(2)D between the lowest and highest quintiles of plasma tenofovir has not been previously reported. Vitamin D binding protein and albumin are proteins that bind to 1,25-OH(2)D, and when their levels are increased they lead to a decrease in free 1,25-OH(2)D (34).

Avatar f tn During those times I had not yet been diagnosed and was able to go back to life like normal, however after the diagnosis and being on phenytoin and switching to oxcarbazepine i am feeling awful. Dizzy, nauseated, moody, extremely tired, severe headaches that come and go. All i get from my Neurologist is these are side effects that will eventually go away. Because i never know when these symptoms will come on too strong I have missed a lot of days at work during my probation period.
Avatar m tn Curious if anyone has any thoughts on the potential efficacy of core protein allosteric modulators?

87972 tn?1322661239 Stephen R Quake3 Abstract More effective therapies are urgently needed against hepatitis C virus (HCV), a major cause of viral hepatitis. We used in vitro protein expression and microfluidic affinity analysis to study RNA binding by the HCV transmembrane protein NS4B, which plays an essential role in HCV RNA replication. We show that HCV NS4B binds RNA and that this binding is specific for the 3' terminus of the negative strand of the viral genome with a dissociation constant (Kd) of 3.
Avatar m tn This reaction is generally confined to the extremities but can affect the trunk and face and may be irreversible. Severe dermatologic reactions to phenytoin are infrequent. Some evidence, however, suggests that erythema multiforme is more likely in patients who undergo radiation therapy while receiving phenytoin.[22] Lupus-like symptoms have been described for phenytoin. Whether phenytoin administration will exacerbate preexisting idiopathic systemic lupus erythematosus (SLE) is unclear.
Avatar m tn My diet is made up of regular meals every 3 hours of protein, fats and carbs to keep hypoglycemia at bay. My question is will there or could there be bad issues having both low blood sugar and high blood pressure at the same time? or are these both two different issues that when combined wont add or increase symptoms?
Avatar n tn Since most of the T3 is bound to protein, the total T3 can be affected by protein levels and protein binding ability, but the free T3 is not.
Avatar f tn Folic acid supplementation in folate-deficient patients with epilepsy changes the pharmacokinetics of phenytoin, usually leading to lower serum phenytoin concentrations and possible seizure breakthrough..." It however says that initiation of Folic acid and phenytoin together is beneficial. Since you can't go back and start all over again, you can take a small dose. It has been observed that as los as 1mg dose can perturb phenytoin’s levels, You may take doses lower than 1mg/day.
Avatar n tn Combining fluoxetine or paroxetine with phenytoin can also lead to increased phenytoin side effects and reduce blood levels of paroxetine. You really need to discuss the potential drug interactions with your own doctor.
Avatar f tn I'm not sure if you're addressing someone specific when you typed "you have stated..." You're correct that exercise can help keep your blood pressure in check, but you have the wrong idea about thyroid "converting energy"... it doesn't work that way. The thyroid produces hormones, T4 and T3... The hormones are what controls metabolism, heart rate, body temperature and other functions.

Limit your intake of foods like fatty meats, full-fat dairy products, and processed foods that are high in saturated and trans fats. Increase your intake of fruits and vegetables: Fruits and vegetables are high in fiber, vitamins, and antioxidants, which can help lower cholesterol levels. Choose lean protein sources: Replace fatty meats with lean protein sources like chicken, turkey, fish, beans, and tofu.
1291453 tn?1272143404 You should tell your doctor if you experience any of these symptoms while taking phenytoin, so that your blood level of phenytoin can be measured and your dose reduced if necessary. " Also, he has had a major surgery, and will need time to recover. However, any changes need to be reported to the doctor.

Avatar n tn Moban® brand of molindone hydrochloride contains calcium ions which interfere with the absorption of phenytoin. Ingestion times of phenytoin and antacid preparations containing calcium should be staggered in patients with low serum phenytoin levels to prevent absorption problems”.
