m trying to find out if I can do my OxyContin instead of oxycodone with the patch, but I would only do 20mg . 2) I put the patch on at 8am this morning and I can't really feel a difference......will it start or does it need more time?

Hi, I was on the fentalyn patch 75 mcg along with percocet for breakthrough pain. I went down to the 50 mcg patch and then went from the patch to oxycontin with percocet for breakthrough pain. Eventually, I got off of the oxycontin and just tapered the percocet down until I got down to 7.5 mg and then went off. I think you can just substitute the oxycontin and then gradually go down from there. I was on the meds for 12 years and went off last December.

I have been in pain managment for 4 year with back and neck issues. I am prescribed Oxycontin 40mg 4x's daily and hydrocodone 10/325 4x daily. Thinking of switching to the patch. Do I need to wean off oxy before using the patch. Do i need to worry about od ing because of the halflife of oxy? So one day I guess they will tell me to stop taking oxy and start the patch i am assuming 100mcg to = what I am taking now. Any help would e great. Pros and Cons to the patch?

When I saw my pain management consultant a few months ago he put me on Buprenorphine Transdermal Patches, 10mg. ! patch to be put on for 7 days. This is in addition to the Oxycontin. The patch has recently been increased to 20mg. I have recently started reducing the Oxycontin in the view of coming off it totally. I have so far reduced it by 10mg. My Dr hase prescibed me Oxynorm 5mg, once a day, incase I experience withdrawal symptoms.

The Doctor had suspected that this may happen. To give to insight, I was on a 75mcg patch and replaced it with 40mg of Oxycontin every 6 hrs. That was not nearly enough. I would suggest that you not do that.

YES I was able to switch and taper from 62 Fentanyl down to 60mg OxyContin. That is quite a bit less OxyContin than the Fentanyl was but it was not hard and I did it all at once in about two weeks. I tapered a tiny bit on the Fentanyl and then stopped it completely and started the OxyContin. I did just fine with little to no w/d symptoms.

Of Diluadid 3xday. Will I have with drawls going from the patch to the oxy? I used to be on OxyContin like 15 yrs ago, I got weaned off of the oxy too on the patch, I had no with drawls from the oxy, my dr. Slowly weaned me, no problem at all. This time, I'm going straight to the oxy from the patch, actually tonight I'm suppose to take my patch off and take my first oxy pill in the morning. Will I be feeling any with drawl? I also take Ativan 1mg. 3xday been on that for 25 yrs.

I am on the Fentanyl Patch and I have tried Oxycontin. So far the Fentanyl Patch seems to be working. As far as the Oxycontin, I have been on it and my body got immune very fast. So as you can see, I have tried about very thing. Pain Management now wants to put in a Pain PUMP. I am really twisted with those thoughts. Does any one have one? This would be equal to your previous dose of Oxycontin before the switch to the Fentanyl patch. If you prefer less than 200 mcg/hr Fentanyl, your doctor could prescribe more medication in long acting pill form and less in transdermal patches. Given incomplete cross tolerance, you may need less of the Oxycontin, Opana ER, or MS Contin (Exalgo is a option as well) than the conversion based numbers above.

While I was in the hospital these past couple of days, they started me on Oxycontin 40mg sustained release tablet. I've previously been taking percocets and I'm finding that, the longer I take them, the less of an effect they have on me, which I understand is totally normal. At the hospital, they were giving me the 40mg tablet every 6 hours and I literally felt in a constant daze - which was fine, when I was confined to a hospital bed!

Hi everyone! As you know I'm finally clean but my hubby is still working on tapering off his pain meds. I think he's to the point now where he can quit the pills completely (taking one a day ... will tell him to do 1/2 for three days then off). He also is down to a 50 Fentanyl patch. Next week he is going to go down to the 25 patch. He's been on the patch for over 10 years. Now the doctor will pretty much do whatever hubby wants him to do as far as getting off of the patch...

I am going really slow and if i need a little I take or put a patch on early.. I made a mistake and took a patch off 3 weeks ago and in 8 hours was in w/d with RLS and aches. It took the patch about 5 hours to catch back up once I got one on.. Again.. my side effects were severe loss of appetite,, contipation was horrible,, had to take something everyday for years.. Depression.. I did not know what the word meant until I got on this patch.. I feel so much better at 25 and plan to drop to 12.

I too use the fentanal patch 100 mc with not much success so far. I was using 160 mg of oxcontin and percocet 10 x 2 for breakthrough pain, when I fell into the coverage gap of my insurance and my prescription was over $600.00 for one month for the oxycontin.

I was actually wondering why your doctor did not try this or another ER medication before trying the patch. I am taking the Oxycontin and it has worked well for me. I also started out on hydrocodone. I believe it would be better for you as it is in a lower class than the ER versions of oxycodone and MS Contin and others in this class. Talk to your doctor about one of these drugs and save the patch for when/if you need a stronger medication than the Fentanyl.

Is there any easy way way to do this and what would be a good option in place of it???? I place one 12mcg patch on every other day and one 25mcg patch. Main reason for stopping is weight gain which is drawing me into major deppression. Any advice is good advice.

I hope it will not make me sick this time since last time I was not taking any kind of pain medication before I started the patch. Now i have taken oxycontin for the last week and oxycodone 2 weeks before that so hopefully the patch will not be such a shock. If an one has any ideas on what I should do it would be very helpful since the doctor doesn't know what to do and it seems as though they think I'm lieing. I am not an addict and am not sure why they are treating me like one.

I was on a higher dose patch (200mcg). I switchted to oxycontin 80 mg, 3x daily, with 15 mg. oxycodone 4x daily for breakthrough, with no bad withdrawals. People in pain like us need the oxycontin 3x daily, despite it's "12 hour" rating or whatever you call it.

s been roughly 2 years and the ulcer symptoms never truly went away thanks to the OxyContin OP formula switch. I had no choice, but to switch to the patch. The MS Contin did cover my pain better then the patch, but I don't have a choice right now. I'm hoping its it too late for my stomach to heal.

Typically a doc will prescribe step down qtys in the fentanyl patch system. Every 4th day you remove previous patch and replc with a lower dosage of micrograms. the patch is a little tricky because it introduces narcotics 24/7. whereas a pill can be sevral times a day. If you have the control to do this you will benefit by tapering your intake by taking less pills or chopping them in halves/qrtrs. Be honest with yourself and you will arrive at the right answer.

I have a general question.... my mother takes 100mcg fentanyl patches and is also prescribed 15mg oxycodone pills for breakthrough pain. The dose for the oxycodone is 15mg but she can take two at a time, up to 3 times daily. That's 90 mg of oxycodone plus the 100mcg patch of fentanyl..... I know that her back pain is real, however this seems like an excessive amount of pain medication. Any thoughts?
