I cannot say, not having been involved in the care of your daughter when she died if it was the methadone or heart failure that caused it. Methadone can cause respiratory depression but also can prolong the qt interval which can cause sudden cardiac death. I can't tell these two apart without reviewing the medical record.

As far as conversion from liquid to pills you should be able to find some sort of conversion chart on line or you could try calling a methadone clinic or Dr. and ask..even if you need to call to a different state. I have in my journal a great article about methadone/withdrawal written by a dr. who was also an addict for 18 yrs. it has alot of info and some things you can do to help ease some of the w/d. If you're taking 300 mg on your take home days, obviously you're going without methadone on some take home days b/c you're taking 2 of your take homes in 1 day. What are you doing on those days w/out methadone? If you're taking pills to substitute the methadone, then you're not allowing your brain time to heal from the obsessions and cravings as methadone treatment is meant to do.

He has been on 130 mg of methadone liquid from a clinic, also has been on 800mg 3 times away of gabapentin both of which he has been on for almost a year. I am worried he will have seizures or worse.
