400mcg/hr is the equivalent of 960mg of oral morphine!!! (a 50mcg patch is equivalent to 120mg of morphine). That is truly an astronomical dose!!! Tapering from fentanyl patches is difficult as they cant easily but reduced in small increments. I woukd suggest trying to taper slowly to about 100mcg (they make a 12mcg patch for tapering purposes) then switch to an oral medication and taper from that.
My mother who has cronic back and shoulder pain is being taken off of fentanyl 100mcg patch and 7.5/500mg and being put on morphine 60mg 2x daily and then morphine sulfate 15mg every 4 hours for breakthough pain. What I'm wondering is there going to be withdrawl or is the morphine going to have enough of the same stuff in it. If not what kind of withdrawl systems will there be to watch for.
During the PM appointment, I had told the nurse practitioner that the fentanyl patches did not relieve my pain. Her solution was to say she would prescribe the same morphine equivalent medication of morphine sulphate, but she wrote a prescription for thirty morphine equivalents less than the fentanyl patches. My pharmacist did not stock the medication and thought she meant to prescribe something for breakthrough pain. When I told him this was not the case, he offered to call the facility.
I went cold turkey. First the morphine and then the fentanyl. I called my family doctor to advise him of what I was doing and he offered his support and yes, he said the same thing, there is no substitute for fentanyl. All he could offer was another opioid.
I have been through withdrawal before so I knew what to expect. The morphine wasn't a major problem but naturally the fentanyl was. I read up on withdrawal symptoms, stopped the patch and increased the water intake to start.
I don't know abt. the patch, it seems like it would prolong the withdrawl. Possibly delay the inevitable but give some relief. Are you withdrawling voluntarily or till your next prescription ? Usually by day 5 a person starts to feel better and alot of the really bad stuff is over. I really do think the fentanly is a bad idea but I know how being addicted goes, we just want it to go away. I've just been switched from Fentanyl 25mcg every 48 hrs to Oxymorphone 10mg every 12hrs. I feel way worse (pain) & was wondering if anyone knows what the equivalence is between the 2. I have a feeling I've just been decreased in pain relief. My dr & the pharmacist told me to wait a few days to "feel it's working" & in the meantime I'm in excruciating pain. Can anyone help with what I can do here? Btw...
gave me the percocets but gave me MS cotin 15 3 x a day. How is that going to compare to what i was on . Also i have had morphine in past an it doesnt agree with me i had reaction to it ! I need to find dr that will keep me on what i was on but how do i find one?
It is not a good idea to take non-prescribed narcs without your doctor's knowledge. A.) It can be dangerous. B.) If/when they find out, you'll be detoxing "really" fast. C.) It will mess up your detox schedule.
It sounds like your doctor is doing the right thing. When reaching higher and higher, escalating dosages of opiates, the ratio of therapeutic benefit to side effects gets worse.

A little more research and I found that the 50 Fentanyl Patch is the equivalent to approx.134 mg of Morphine (and of course that is what the Ms-Contin is). So it's no wonder that you are in so much pain. I sure hope that you can get some help from him tomorrow.

Why don't you try a nice hot bath or shower as that might relieve some of your pain. Wish I could help you more.
ve been using a lot because I already have a high tolerance to opiates, what I wanted to know is if withdrawal symtoms of Morphine are any worse than Tramadol, because I think I may need to progress to that eventually.

I detoxed of 75 mcg every 48 hours of Fentanyl. All I can say is go VERY slowly. What kind of patch do you have?
Fentanyl withdrawal is a bit different than other opiates. Not as bad as methadone but pretty tough in it's own right. But it can be done.
At times I tried to go too quickly and that was a huge mistake. follow the Thomas Recipe and above all, keep yourself as hydrated as possible.

So I'm on the Fentanyl 25mcg patch, switched from 180mg of Morphine ER because of tolerance. As I've said here before, but reiterating for those who may not know, it doesn't work for me. The Fentanyl that is. It was pointed out that the dosage is most likely low and I should be on the 50mcg patch, but I'm not a big fan of the patch in general.
