When you eat, you need the fast acting insulin like Novolog to act quickly to cover the meal. For some, Lantus and Levemir act the same. For me, Levemir seemed to work better in that it provided more consistency to my blood sugar levels. I like Levemir. I have switched to 2 shots per day (morning and night) instead of just taking the 1 shot at night, because the 1 shot at night may not cover the 24 hours.
When I started using 2u Lantus in morning, 2 weeks ago, I'd go low in the late afternoon/early evening, and then my morning sugars would be elevated ... 140s, 150s. So I split the dose: I'm now using 1.5u Lantus AM & 1u Lantus PM. It definitely works better than 2u QD, and I've checked the middle of the night - not a problem. The drug sheet for Lantus doesn't mention BID dosing, but my internet articles do. What are your thoughts, and is Levemir better for BID dosing?
It was working so well for me previously. I went to a new endo about a year ago, and he suggested I try Levemir instead of the Lantus. Once I started the Levemir, I noticed a marked improvement with more consistent bg levels. I am a Type 1 diagnosed at 33, and I am 10 years into the disease. I take sliding Humalog and 12 U Levemir at bed. I hope this helps.
My daughter seems to be having the same problem. She's 15 and takes 68 u of lantus at nite and supliments w/novolog pen when she's eating. They just keep increasing her dosage, but nothing seems to change. She eats pretty decently (for a teen), but she always seems to be high.
I also wondered if she might be resistent to the lantus. We've tried new bottles, etc. How many units do you take per day?
About a month ago, my doctor changed my long acting insulin from Novolin NPH to Lantus. This completely threw me off and I ended up being woken up by paramedics 3 times in 1 week. My blood sugar would be normal before bed and then by morning it had gone to 1.8. When I would eat a snack before bed to keep my blood sugar from dropping, I would go high - 13.0-15.0 on average. My doctor has now switched me back to my Novolin NPH and I am still having a hard time getting back on track.
If you believe your are reacting badly to Lantus (or any other medications) you should ask your physician for an alternative, at least to verify it is the medication and not some other factor that is causing the problem. Lantus is popular for it's 24 hour action, but so is Levemir (Novo Nordisk). NPH insulin can also be used for long acting insulin, though two injections may be needed to get the 24 hour coverage.
What was she on before and what were the results? If she is experiencing lows or highs in her fasting and in between meals and bedtime than I would adjust the Lantus accordingly up or down. Always adjust a very small amount initially and see how that works for a couple days. If she is experiencing highs or lows two hours after meals than it would be her bolus and her Insulin:Carb ratio needs tweaking. You need to give a bit more information for us to go on.
I am on lantus and the Novolog pen. I know when I first started lantus I got very anxious and before I had to take it, just in a dead panic. I can not fully answer your question, but I need your help. Can you ask your daughter if her lantus burns, itches, and/or does the area she injects it (arm, leg, stomach) feel heavy or like a big knot is under it?
I read some articles about this just yesterday and was surprised, for I take Lantus and have noticed no weight gain at all. So this could be a factor.
However, a change in metabolism in middle age probably is the real culprit, along with possibly a slower metabolism because you are not eating. Exercise should help both problems. Start walking! Park your car further from the door, take stairs instead of elevators, etc. Or use your lunch hour to do some mild exercise right in your office.
NPH and regular Humilin are older therapies. Something like Lantus or Levemir as the long acting and something like Novolog or another fast acting insulin are much more common these days. They give more consistent coverage with fewer spikes. Does he have hypoglycemia unawareness? If he's getting down to 19, it sounds like he may. That is a condition where he can't tell he's getting low. It is possible to correct the condition, you just have to work out a plan with his endo.
I take 40 units Lantus (long acting), and sliding scale Novolog (fast acting) insulins. My bg is always so high in the mornings, I have to take Novolog and wait for sugar to come down to eat. I've had diabetes for many years, but never knew for sure which one! My dr would switch my diagnosis all the time. But went to endo and they did a blood test, and said I'm type 1. I'm angry my dr. never did anything about this.
Alternatively, talk to you dr about using levemir. This is dosed morning and night and the dose can be different for morning and evening to target optimal blood sugars. There is also a new insulin called Tresiba. Many find that this gives very stable blood sugars with less risk of lows. You may also discuss this with your Dr.
Hello. I am T1 since Oct 2004, and switched to Lantus in Feb. Most of my BG numbers are very good (A1Cs 6.0 and 5.5), but since switching to Lantus, I have experienced 5 episodes of sudden and extreme drops in BG. They happen late morning, not related to breakfast, novolog dose or exercise. My BG is usually above 150 when it starts, and I must consume 150-200 grams of carbs (as juice or gel) to get it to stabilize, usually in 20-30 mins.
I have been on insulin for nearly 38 years and am lucky enough to have no complications so believe I have managed my control pretty well. However since starting to use Lantus (and I was one of the first users in the UK) I have stopped having real problems with hypos but have had great problems with consistency of delivery. I want to find out whether this is just me (and I do not think so) and how many other diabetics are having similar problems and what they are doing about it.

To me, the basic difference is variability and flexibility. With Lantus, you know exactly what you are dosing, and you also dose exactly the amount of fast acting you need. With 70/30 there is a lot extra to take into account. I personally don't know many, if any, people who are still using mixed insulin, except in poorer countries where they can't get anything else.
Hi. I'm not a medical professional, just the parent of a kid with diabetes. What insulin is he using? If he's not using NPH as his long acting insulin, skipping meals isn't as big of deal. Something like Lantus or Levemir as the long acting would work fine, then for meals when he can eat he would just give a bolus shot with Novolog or some other fast acting.
I use a short-acting insulin, Novolog, and a 24-period insulin, Lantis. I give Novolog with each meal, and Lantus when I go to bed. I am having troubles with my sugars in the morning. I wake up at 7am, and eat breakfast. I give my shot immediately after eating breakfast, around 7:20. I test my sugar before I eat breakfast and it is normally good. Then at 8am, I head to school. When I arrive at school around 9am, I test my sugar, which is usually pretty high (200-300).
