Well this can happen with ocd, but I think it happens with a few other disorders like anxiety too. It usually takes more than one symptom to diagnose ocd, so I'd get a second opinion. Also when you say "therapist" do you mean a psychologist, a behavioral therapist, or a psychiatrist. Only dr like a psychiatrist or pcp can make a diagnosis, so if it's someone who can't prescribe medication, I'd find a new therapist.

At 200mg. your at the highest recommended daily dosage.
Although Zoloft is not the worst offender, you can probably expect an increase in appetite and weight gain. This, unfortunately is a typical side effect of most SSRI's.
However your other habits point towards an obsessive compulsive disorder. You need to consult a psychiatrist regarding them and go through behavior counseling regarding this.
