If not 8am fasting, it was not useful... And you don't have complete loops (cortisol and ACTH, estrogen and LH,FSH, DHEA-s, bioavailable testosterone, GH and IGF-1 etc). Plus sodium and potassium and renin and aldosterone are needed to know if you have low cortisol followed with a stim test.
Avatar m tn So if testosterone is low, other things are working overtime high to make up for it or not functioning at all - so the loop is broken. And since the androgen can come form the pit or the adrenals, they need to test you well. If you are low, and just replacing to the point of feeling better, your side effects should be reduced - talk to your doc or pharmacist about your risks.

Avatar f tn High AM cortisol, High PM cortisol = Cushing's syndrome. Even if you fail the stim tests (I did) and the dex tests (I did), you may still have it but the doc, who may not understand Cushing's, may not know how to diagnose it since it is not a single test yes/no thing. There is no OTC way to lower cortisol. You cannot lower your female hormones and lower cortisol. Progesterone CANNOT turn into cortisol.

Avatar m tn Your doctor needs to do a LOT of other testing - including more cortisol testing like 8am cortisol serum, more 24 hour urine testing, midnight saliva testing, plus all pituitary, thyroid, androgen and all the rest of the adrenal hormones. That would be basic and a lot of the tests would have to be done over and over just to show a consistency.

ACTH is the pituitary hormone - that is the hormone that the pit sends to the adrenal to signal the adrenal to put out cortisol. So you need to know if both are high, one is high, etc. to know where the issue is. That is great that he is doing a lot of tests, but he is not able to put the tests "together" to a logical conclusion and treat a person if he wants to wait.

Feeling wired, excess hair, lean body, irregular periods and excess testosterone production all seem to point to it. Get your testosterone, free testosterone, DHEA and androsteneidione tested- they vary depending on where you are on your cycle, so ask your endo when you should get them tested.

Avatar f tn I was diagnosed last week with adrenal insufficiency (the ACTH Stim test results aren't back yet, but based on the Cortisol test and ACTH test, I presume it's secondary) and given my medication. I think some of my symptoms are from the hydrocortisone and not from the AI. How am I supposed to tell the difference? Other than getting the flu or having a surgery, when do I stress dose? Do I need to do it for exercising? I take 15 mgs in the morning and 5mgs in the early afternoon.
Avatar f tn The body hurts a LOT when cortisol is low - cortisol is an anti-inflammatory so it could be a factor here - and you have to fight to get help. Doctors are not good with this stuff. Links in the health pages may help.

In addition, the master endocrine organ i.e the pituitary also needs to be investigated. You need to consult an endocrinologist and get it evaluated. Avatar m tn In any case, for either disorder, one set of testing is not and should not be used before you are placed on any type of replacement regimen. If you do have elevated cortisol and you ADD a cortisol agent, you are adding fuel to a nasty fire and will only make your symptoms worse. Cortisol normally varies - it is supposed to. It is supposed to be higher in the morning to wake you and lower at night to put you to asleep. You only had one test, one day.

It is unpredictable if your adrenals will keep going, or will stop. Myself, I had high cortisol for over 12 at least, and when my adrenals were removed, they were enlarged and nodular. However, my CT scans both before and after surgery said my adrenals were normal.
