Zonegran is awesome! It really has worked great for my daugther with little to no side effects. Initially, there was some problems with being a little sleepy during the morning, but that wore off. It also did cause some appetite suppression in the morning, but that wore off too.

How was your MRI and tilt table study? Topamax may cause side effects such as muscle spasm or shaking while Zonegran may also cause chills, tiredness or body aches. If these persist, it is best that you inform your attending physician for proper management. Increase in blood pressure may be caused by a variety of issues as well such as underlying condition and from physiologic causes like sickness or from physical exertion. Regular Blood pressure monitoring and low salt diet may also help.
There are even some meds, especially antidepressants, that cause weight loss in some and weight gain in others. I gained weight on most mood stabilizers. The biggest culprits for me were Depakote, Remeron- an antidepressant, and Zyprexa- an antipsychotic.
Now I take Lithobid, which is a slow release lithium. It tends to not cause as many side effects as regular Lithium. I am also on Seroquel. I was finally able to lose all of the weight I gained.
I used both, and found that side effects are similar; tiredness, weight gain, and a slow pulse. Side effects are more pronounced as the dosage is increased. I had dosages from 25mg to 200mg. daily. Other reported effects are loss of libido, tightness in the chest, itching, irregular breathing, and irregular heartbeat. As halbashes said, it would be important to avoid anti-epileptic drugs that sometimes have the side-effect of weight loss or loss of appetite. There are others that tend to cause weight gain, though, such as Depakote (which is also used to prevent migraine headaches). A good neurologist should be able to choose the right drug with the right side effect profile. Obviously, weight gain and/or increased appetite would be desirable.

I have back problems and the extra weight was brutal. Soon as as switched to Topamax (which has weight loss side effects) I lost the extra weight plus.
Seizure meds are tough. They all have very extreme side effects. I've been on 4 different ones and I found that over time my body would get used of the side effects. Depakote was definately one of the better ones for me.

If it's the latter there are a few practical steps that can help: cold food vs. hot, many small meals vs. a few big meals. Although it's damn difficult, he needs to try to do everything possible to keep his weight up.
