Avatar f tn how can i get records from a mental hospital my boyfriend was in that has since shut down
Avatar f tn I'm a 16 year old female... I have been experiencing muscular pain, deep pain (feels like it's in my bones), fatigue, headaches, muscular spasms, and loss of appetite. I went to my doctor and had blood drawn. Everything came back normal besides my vitamin D levels- my doctor informed me I'm vitamin D deficient and according to previous records, I have been for years.
Avatar n tn These records are legally yours for you to keep for your own personal records. Trust me, if you end up with something serious, you'll be glad you have them on hand.
1065668 tn?1256319504 I would recommend sending your records to Dr.Norman -- the medical records place from your doctors office would probably fax them--- It does seem that your parathyroid is over active-- have you had calcium levels done? Have you thought about goin to a thyroid specialist like Dr. Lupo in sarasota flrodia?

Avatar m tn Sorry my meds kicked in and I didnt get to finish my post.. Does the vitamin d and b account for chronic pain? I have been going to my doctors for possible ra and or fibro. I have been told I have both from my rheum doctor and then told I had neither that I need to see a muscle skeleton doctor and or a nurologist. I have both upper and lower body pain and have allways had bone pain as long as I can remember. I also have add or adhd, I am 32.
359574 tn?1328360424 I want to work in medical records when I grow up. They are never there, and apparantly, they don't ever have to answer the phone or return voice mails. I called my PCP's office to get copies of the few things that have been done since she first sent me to the neuro. They have a good excuse. Their calls were being routed straight to voice mail, and I filled out a form and got the job going. I'm lucky, her office is 2 blocks from my house.
Avatar f tn Two weeks ago I filled out a form at my pcp's office to have my entire set of records. I used to go to a Woman's center before I went to this pcp, and I'd had my records transferred over to the pcp. So I figured my entire life history would be available to me. I could use all of this to spread out on my kitchen table and see my medical history unfold before me. No. I got all my lab reports, MRI's, CT scans, but not the entire enchilada.
Avatar f tn Are my medical records private? Will my employer find out if I have been exposed to HSV? This should be the least of my concerns at this point, but I work in a small office and I am terrified that they will find out.
Avatar f tn Has anyone ever paid for the medical records and sent it to the new office and not get approved by them?
Avatar f tn However, I’d like to share something…in the past we have always stressed the importance of having your own medical records, and I would like to stress that with everyone again. Last week I traveled to the state where I once lived, where I believe my medical problems began. My current immunologist was hoping to connect my current condition to a virus I had years ago. The medical records I obtained from my old HMO include EVERYTHING.
1137779 tn?1281542505 First of all they tried to tell me that they routinely scrapped all records older than 7 years (yet this was well within that time), so they had probably 'routinely' scrapped my scan. All things considered, chronic runarounds included, this seems to be extremely suspicious. What would you folks think? samxx PS £3billion is the yearly budget for the health service's defending-the-indefensible department.
Avatar f tn I passed out at Walmart 30 minutes after leaving my pain management doctor who had just refilled my pump. I now get my hospital records and they have it looking like I was an alcoholic druggie!!! I do not drink!!! They claim there was alcohol in my blood but not much. They also said I overdosed on fentenyl. No mention of pain pump implant possibly malfunctioning. Is there anything I can do? I do not want this on record because anywhere you go they ask if you have been hospitalized.
646318 tn?1261181494 I always hear people on the forum tracing back their medical records... How do i go about doing that?? Do i actually go to every doctor i've been to since i was born?? which is pretty impossible..or is their a better easier way of doing this??? and is their a way to trace back records from my father who has been deceased for over 15 years?? maybe this should be on the social forum, so sorry if this is on the wrong forum...
1858011 tn?1319837353 When you ask your doctors or hospital for your records how do you really know if they are giving you all of your records? And what if they have a problem giving your records to you? Tks misty.
Avatar f tn so I posted that I had switched doctors and it will have been 3 wks since my appt with the new doc and they still have not received my medical records from the old doctor so I can register at the new hospital I plan to deliver at has anyone else had this problem it may seem small but its really frustrating me especially since I have history of preterm labor...
Avatar f tn You may need calcium supplements to prevent bone disease and vitamin D to maintain calcium balance. Iron supplements may be needed if there is anemia. For more queries consult your doctor since he knows your medical records best and the associated risks also. Hope it helps. Keep me posted. Take care and good luck!
Avatar m tn Well Barb, I appreciate the imput. I'm new to forums, so I'm learning. My last test results showed: TSH 0.02, T4= 1.07, T3= 3.49, B12= 149.T4 was at 1.38 few months ago. I didn't see Vitamin D levels (not sure if there's another term listed) , but I have been taking 2000 units daily for severe deficiency. Also been on iron and cyanocobal (100mcg) for anemia. Although I've mentioned what I'm going through, my records list me as asymptomatic.
Avatar n tn Goolara pretty well said it all--- I am hypo/Hashi and have both low vitamin b12 and vitamin D. I've been on b12 shots for over 2 years (should have been a lot longer) and just recently found out about my vitamin D deficiency, so have not started a treatment for that yet. In fact, that reminds me that I need to call the doctor about that............................lol I'm finding that both vitamin b12 and vitamin D deficiency can cause fatigue just as hypothyroidism can.
Avatar f tn ve been having, my doctor decided to check if I was vaccinated against hep B, even though my medical records say that I was. According to the blood tests, I am not immunized against it anymore, and I'm wondering how that could have happened. My complete blood count, b12, folate, vitamin D, and thyroid (those were the other things she checked), are all normal. Any ideas how this could have happened or what it might mean?
Avatar f tn Thyroid Perioxidase (TPO) Ab 131 IU/mL (High/0-34 limits); Vitamin D, 25-Hydroxy 9.5 ng/mL (Low/32.0-100.0 limits). My records noted I had a small goiter and am experiencing a choking sensation (golf ball) primarily on the right side of my neck. I did not note a TSH test in my first round of lab tests but upon a subsequent visit to the doctor was told my TSH levels equaled 2.69. The only treatment prescribed was a high dose of Vitamin D. The choking sensation was not addressed.

Avatar m tn She thinks I might have fibromyalgia with a vitamin D deficiency. Really? Is it possible those two things could explain it all? Oh wait, fibro, vitamin D deficiency and migraines. I think that's where this is heading. Or maybe I'm just in a bad mood for meeting with a doc that was going to run a screening test that comes up negative every time. I had to ask for the antibody tests instead of the ANA screen. She said I don't look like I have lupus.
502909 tn?1212072396 He looked at my records from the hospital and said the same thing they did. Nothing is wrong with you! Ok so long story short he Prescribed me Celexa (20mg daily) for my anxiety disorder. My question is, has anyone else ever taken Celexa? I never have. I am also on a water pill, vitamin B12 tabs, and a Beta blocker for my Renal Hypertension. I just wanted to see how any of you felt about it before I fill it. Thanks a bunch!
