Have you ever had sex with a woman? Possibly you just lack the experience. Possibly you need more foreplay time.

Blood pressure and heartrate may well be connected with this problem (the penis becomes hard by being pumped full of blood). Possibly being underweight too, I don't know about that. Viagra and surgery can both help. I'd certainly try Viagra before even considering surgery!
