I am 27, and have always suffered from premature ejaculation. I tried Fluoxetine in the past and found it useless, I am currently on 100mg sertraline daily. After 3-4 months on sertraline I've noticed a slight improvement that is still hit and miss, have tried all sorts of "start-stop" exercises and kegel exercises for nearly a year. I also have occasional anxiety-related ED problems which I take Levitra for. What is my next step?

Usually SSRI's decrease premature ejaculation. In fact it can make it downright difficult for some to ejaculate at all. Most 20 year old males get exited quite easily, so don't worry about that. it will most likley decrease with age. As one who suffers from General Anxiety Disorder and severe panic attacks (and depression) from my experiences, you have to learn to try not to worry about everything under sun. Get checked by a Doctor if you need to, but most likely everything is just fine.
I also last about 10 - 40 secs during sex (premature ejaculation) and I have a very hard time trying to urinate when I know other people are around. I also seem to have a really high sex drive and I get aroused very easily. My erections come on very fast. I would like to know if SAD could be linked to these problems? I'm not on any medication currently. I tried Paxil and Zoloft (low doses for about 4 months) for my anxiety but neither of them worked.
