tacrolimus vs sirolimus

Since I was on tacrolimus only(as Mycophenolate causes TLC to go down in two occasion), sirolimus 1mg is started along with reduced tacrolimus(2mg BD, Tac level ~4.5). But the creatinine is still high(1.8). It seems some side effect of Sirolimus is also coming as the TLC is 3400 and platelet is 80,000. What do you suggest to bring creatinine under control? What is the way to determine the exact required dose of tacrolimus?

there are prograf assistance programs that the company offers. If this is not feasible than using other cheaper medications such as verapamil for example for HTN for example will alllow much lower doses of prograf to be used (it delays metabolism of the medication) and thus it comes out cheaper.

i would be extremely cautious about using it until more data is available especially since its purported effects on tacrolimus and cyclosporine metabolism are extreme. i am quite confident that there will be clinical trials coming down the road extremely soon.

My concern is whether the rest of me will make it a lot longer with this same regiment. Is switching to sirolimus something to seriously consider? Does it use bile acid to metabolize making it a bad match with someone having to rely on ursodiol to keep bile presumably flowing adequately? My bilirubin avg lately 1.3.
