My 3yr old chews on the shoulders of his shirts leaving holes in his shirts, when his coat is on he chews on the front of his coat, I can't get him to stop and I'm worried about his health, he barely eats and half the time refuses to eat, he just wants fluids. I'm scared for him, I also have a 6 year old Autistic son, I never went through this with him.
It is important that you have to rest the injured muscle and modify your movements during changing of your shirts and t-shirts and do not exert the particular muscle before it heals.
Presently you apply ice packs, take pain killers and wait for couple of weeks for your symptoms to decrease.
If symptoms continue then you have to consider MRI scan and a follow up with an orthopedician.

Does he also use Delayed Echolalia eg. pieces of dialogue phrases or words from TV or films? The Speech and Language Therapist should be able to tell you more about it. I would specifically be asking for a Speech and Language Therapist who also has experience of autism.

He just completed his k5 year and has been in speech for two years. The speech therapist believes he has a language based delay. K5 was very difficult with the phonics approach. After 6 mos of trying to teach him the alphabet and sounds with little success, I bought a leapfrog DVD. Surprisingly, within a few weeks, he knew ALL of his letters and sounds. As far as going to blends, it was an epic fail.
My 9 year old son has a serious oral fixation. He chews his fingernails, chews his toenails, picks his nose & eats it, eats the collars on his polo shirts (he doesn't just "chew" on them, he actually "eats" the material....& there is no collar left) & chews holes in all his t-shirts right below his chin.
At 18 months we took him for evaluation and they scored him on the low side of the communication, language category etc. So a speech therapist started coming to our home and in 6 months he didn't make much progress. She tried to teach him signs for "more", "give me" etc but he is not interested.
After 6 months into therapy they evaluated him and the result was inconclusive.
He didnt babble much as a baby, and not very much as a toddler, so at 2 years old I took him to a speech therapist...........now he is 4 and a half and the speech therapist just noticed that at the back of his throat his uvula is doubled, and she felt the back the top of the roof of his mouth and told me he has Submoucus Cleft Palate. We since have seen a nose ear and throat specialist and he says that my son doesnt need surgery, and should out grow his nasily sound by puberty.
She was assessed by early intervention (psychologist, speech therapist, and occupational therapist) and her pediatrician for autism and no one had any concerns. The speech therapist said she should be evaluated by a neuropsychologist. This is a very expensive assessment. Given the fact that she has been evaluated a couple of times do you think this is necessary? I am so frustrated, I have been trying to get her speech help for so long and have jumped through so many hoops.
I work with development delayed children if you are seeing a therapist with your child the best thing they can do for you is be your voice with the doctors we have a speech therapist that gets things done with the doctors tell the therapist all your concerns that is the best thing some parents think maybe they sound like they are complaining to people please don't think that I tell everyone you don't know unless you ask questions
