As others here have commented, a real psychotic episode, which is a very serious event, must occur in order for their to be a true 'bipolar I' diagnosis. The diagnostic 'bar' for labling someone as 'manic depressive' must be high, because of the risks of the medications that are needed to control this illness, among other factors.

He was diagnosed with Major Depression with psychotic features. He has been on 20mg of Zyprexa, 100mg of Wellbutrin, 60 mg of Prozac for three years. Is this a normal course of treatment?

I would not worry too much about getting the right 'label' as the treatment is basically the same for both conditions; an antipsychotic and mood stabiliser. The difference between BP1 and BP2 is that BP1 has more Manic episodes with psychotic features/hospitalizations or have placed the person in danger. BP2 are more likely to be hypo-manic fewer full blown manic episodes. It doesn't matter both are very difficult, can be life threatening and treatable. It's important to get treatment as early as possible if possible and if stable try to stick with a plan that is working.

I know of four bipolar 1 with or without psychotic features, bipolar 2, bipolar nos, and cyclothimia. With bipolar 1 you get full blown mania and major depression, with bipolar 2 you get hypomania and major depression. cyclothimia fluctuates between minor depression and hypomania. Bipolar nos is where they put you when you obviously show some symptoms of bipolar disorder but they do not know which classification to put you in.

I have schizoaffective but from what you are describing you might have bipolar with psychotic features (ask your psychiatrist). But I had psychosis all the time as is typical for schizoaffective but with bipolar with psychotic features when you don't have moodswings you don't experience psychosis.

Wow you guys are waaaaay off melancholic depression is a more severe form of depression which is psychotic in nature. It gets it's name because it is depression coupled with an extreme apathy and inability to feel joy when say complimented etc. It's always worse in the mornings. I highly recommend that if you think you have this you speak to a dr immediately as some people (myself Included) suffer from psychotic episodes of intense fear and rage.

Depakoate is a mood stabilizer. Zoloft is an anti-depressent. Seroquel is an antipsychotic and I believe by psychiatric standards that is a subclinical dose (when I was on it 300 mg. was where it started working). Not sure what "the pill to calm her nerves" is? You mean an anti-anxiety medication? Although only a psychiatrist can determine medication dosages and which medication to use I'd suggest you inform yourself.

It sounds like he possibly has severe depression. Some people with severe depression also have psychotic features (depression with psychotic features). It could be worth considering what happened a month ago to trigger this current state. I think encourage him to see a psychiatrist and perhaps look to follow that up with psychotherapy. You could ask him if he would like you to attend his appointment with him as a support person, etc.

i think the differnce is your more active with major mood swings and depression you sink more into your self don't quote me. if i were you i would speak to her or him again who you wen't to see and ask them to explain it in english please in terms you can understand best of luck and take care lynne.
