He was diagnosed with Major Depression with psychotic features. He has been on 20mg of Zyprexa, 100mg of Wellbutrin, 60 mg of Prozac for three years. Is this a normal course of treatment?

When I was younger before my diagnosis I had similar psychotic features and I was treated for major depression with psychotic features. I was treated with anti depressants and anti pyschotics. Unfortunately the anti d's triggered rapid cycling and lithium was then added to the mix. If it helps I no longer hear voices, it stopped around age 20. Now during an episode I have classic bipolar features.

With bipolar 1 you get full blown mania and major depression, with bipolar 2 you get hypomania and major depression. cyclothimia fluctuates between minor depression and hypomania. Bipolar nos is where they put you when you obviously show some symptoms of bipolar disorder but they do not know which classification to put you in. There is also schizoaffective disorder which can have some symptoms simmilar to bipolar disorder.

I do know that for schizoaffective disorder delusions and psychosis occur at all times whereas in bipolar with psychotic features they occur only during moodswings. You can speak to your psychiatrist about this but also keep a mood tracker and print out the results for your psychiatrist as that can be markedly helpful as well.

There are a variety of specific types of bipolar, some where mania is the predominant feature and depression less so. Some mood stabilizers specifically treat mania better than depression so you could ask your psychiatrist about that.

Bipolar has no aspects of psychosis. With bipolar with psychotic features, during a period of moodswings (mania, depression) a person has auditory hallucinations (hears voices) or visual hallucations or has delusions (perceives things that are not in reality). Even with current treatment the symptoms can be mitigated. He should have his psychiatrist follow up as to an exact diagnosis and when the onset of it was.

Wow you guys are waaaaay off melancholic depression is a more severe form of depression which is psychotic in nature. It gets it's name because it is depression coupled with an extreme apathy and inability to feel joy when say complimented etc. It's always worse in the mornings. It sounds like he possibly has severe depression. Some people with severe depression also have psychotic features (depression with psychotic features). It could be worth considering what happened a month ago to trigger this current state. I think encourage him to see a psychiatrist and perhaps look to follow that up with psychotherapy. You could ask him if he would like you to attend his appointment with him as a support person, etc.

With severe depression you can have psychotic features. Also, with bpd, when you get extremely stressed you may experience transient psychotic stuff too. This is short-lived and you should be OK at reality testing (which makes it different from other psychotic disorders). You could try looking the diagnoses up in the dsm-iv. That may help you better understand what the disorder is comprised of.

I had a psychodic episode five months ago: and at first they gave me anti psychodics. In five months doctors have tried the following. Three separate hospitalizations. s not generally a part of depression but it can be from depression with psychotic features or other related conditions but only a psychiatrist would understand the specifics of it but ask them where the paranoia comes from. Recovery rates from antipsychotics vary but they will improve as new treatments become FDA approved (you can read through my posts for more information) so don't give up hope. You have to remember that there are times where people with bipolar are euthymic (normal) in between moodswings and it sounds like you are a rapid cycler like me.
