Avatar n tn is wanting to replace the Duragesic and Percocet 10/325 ( which I take 3 times daily, 90 per script) to Oxycontin. what Mg.. of Oxycontin would be required for me too take being that Ive been on pretty high doses of Duragesic and Percocets for years now?

Of course they don't have a crystal ball but discussed switching me into another medication and spoke of the conversion table they use to determine what dose to use. My question is this....
Avatar n tn My doctor reduced me to the above dose and its going terribly. The conversion for the fent for that dose is 50 to 75Mcg. My dr said to try changing the patch every 2 instead of every 3. My insurance won’t pay for this non-standard dose. Idk what to do. I’m in constant pain, sweating crazy - i’m in a nightmare.
Avatar n tn All I know is that they will use a conversion chart to do that transition. I asked the PCP what to expect, and she said that I would feel some discomfort for the first couple of weeks but that it wouldn't be bad. She really wasn't convincing though. I have been on 75mcg/q48hrs for 4 years. I am sure that I am very dependent on this stuff. Why all the fear you may ask??
Avatar f tn I've been on the Duragesic 100 for almost 18 years. I've had 3 cervical fusions & 1 lumbar laminectomy. The Duragesic 100 has controlled my constant, chronic pain & while I've never been pain free, the Duragesic certainty helped. However in the last 2 months, its as if the patches are no longer working. I've always had additional breakthrough meds, but now its as if I'm going through withdrawals.
Avatar m tn I have chronic pain from Tarlov Disease and Interstial Cystitis. My pain has been well managed for 2 1/2 years using tramadol (ultram) for maintenance and lortab for break through pain. I have developed chronic kidney disease and my doctors are pushing me to get some exercise. I have found that with every exercise form I have tried I quickly get beyond the pain control offered bu tramadol and lortab so i can't exercise.
Avatar f tn not exactly..oxy in theory is a bit stronger than hydro..and sub is a different type of narcotic than the other 2..but conversoin charts do compare the strength of sub to other narcotics..but it is not really the effect they r comparing//but the strength...i do believe there is a conversion chart in avisg's journal...
Avatar n tn I have had an addiction to oxycontin and went to detox and rehab. I am 11months and 5 days clean and sober and am enjoying life being sober!!!! I was given dilaudid in the hospital and I have been sent home on the Duragesic Patch. I had full disclosure with the doctor and he knows my entire history. Unfortunately, the pain is to bad to just be controlled with over the counter meds and I was told that Tylenol will just aggrivate the situation. Needless to say, I am scared to death.
Avatar m tn Hi, I was taking 30 mgs of oxycontin per day. I stared the fentanyl patch 5 days ago. After a couple of hours, I felt it had some effect, mostly a mental type of euphoric feeling. But then after, I´m really not sure if it´s working. I´m on the second day of my second patch. Wanted to know if this is normal? Could it take up to 5-6 days to feel a full effect from them.
725248 tn?1316162245 I will also be talking to my pain doctor about the pain pump at my next visit. I am currently on Duragesic patches 100mcg and oxycontin and they are not helping at all. I've been thru PT, 6 epidural injections, tense units, and nerve blocks with no relief. He had talked to me about a spinal cord stimulator, but after viewing my MRI said that my spinal canal is to narrow due to having a two level cervical fusion.
Avatar f tn I am on the Fentanyl Patch and I have tried Oxycontin. So far the Fentanyl Patch seems to be working. As far as the Oxycontin, I have been on it and my body got immune very fast. So as you can see, I have tried about very thing. Pain Management now wants to put in a Pain PUMP. I am really twisted with those thoughts. Does any one have one? How is it and how does it work for you?
Avatar n tn I have been going to pm for a little over a year now and have been using the duragesic patches for the last 4 months.The patches are a godsend and i can maintain a 4-5 pain scale.You do not mention a sleep aid or a muscle relaxer as part of your meds. I consider them both mandatory for this curse.
Avatar f tn I spent 15 yrs taking oxycontin, duragesic patch (fentanyl) for previous back surgeries. I still had continual pain. 2 yrs ago I decided to stop all pain meds. My body was physically addicted and built up a tolerance. I never abused my medication and took it exactly as described. Initially, I tried to wean myself off-stopping one of the oxycontin's per day. The withdrawal was "nuts". My legs thrashed, I had freezing cold sweats, shivering, agitation-and God knows what else.
Avatar n tn To get dosing equivalent to 120 mg of Oxycontin through administration of MS Contin, you should have been prescribed 180 mg of MS Contin based purely on the pharmacological conversion factor between Oxycontin and MS Contin as Oxycontin is 1.5 times stronger than MS Contin.
Avatar m tn Taking 15 mg of MS Contin 3x a day is equivalent to taking 10 mg of Oxycontin 3x a day. I hate when doctors sneak in a significant dose reduction through a conversion to another opioid! Even factoring in a cross tolerance factor, your dose should have been at least 30 mg 3x a day. But to get the same strength you were getting from the Oxycontin through MS Contin, the dose of MS Contin should have been 45 mg 3x a day. Expect to experience increased pain and withdrawal symptoms.
Avatar m tn Have you ever tried a Duragesic pain patch? It is strong (in diff doses)and steady and lastly about 3 days if you've got good skin absorption. The med in Duragesic is about 100 times as strong as morphine mg for mg.
Avatar m tn t know where else to put this, but I was recently switched from Morphine 60mg ER to the Fentanyl Duragesic patch. I thought the weird flu like symptoms might be a result of the new drug, but apparently it has to be withdrawal from the Morphine. I'm kind of miffed neither the doctor nor pharmacist even mentioned this could happen. I was naive and thought the new medication wouldn't create an issue with the switch, I was wrong.
Avatar f tn I have been on Fentanyl for years now and as a single mom I can say it took my pain away and didnt make me high (which I loved because I was able to care for my children.I will tell you it helps you sleep alot better and I really didn't have any problems transferring I went off oxycontin and onto Fentanyl.For some it is a lease 0on a new life.
Avatar f tn That is not the case if you taking the 80 mg and the 40 mg 3x per day. That is 120 mg x 3 or 360 mg of Oxycontin per day vs. 120 mg per day. Let me rerun the conversion calculator... So if you remember I converted your total daily dose of Fentanyl to Oxycontin and it came out to be 210 mg of Oxycontin. That is 150 mg of Oxycontin less than what you were taking through Fentanyl vs. 90 mg of Oxycontin more than you were taking.
Avatar f tn What can anyone tyell me about the opana conversion? Here is a little background. Within the last wo years the great Oxycontin change took place. The FDA produced a version of the Oxycontin pill that helped prevent abuse by making the pill extremely difficult to grind into powder form, to be snorted or otherwise. It seems the3 FDA has done the same thing with the Opana. I am looking for any and all information that can offered. thank you.
Avatar f tn i have been on percocet 10/325 for nearly 4 yrs and my ortho added 2/20 mg oxycontin 2 months ago because the percocets don't last very long anymore. i've got bone fragments in my neck & a lower back injury which caused total paralysis twice (temporarily) a few years ago & am on disability for such among other illnesses, which may or may not include scleroderma and/or R.A.
Avatar m tn I have been in pain managment for 4 year with back and neck issues. I am prescribed Oxycontin 40mg 4x's daily and hydrocodone 10/325 4x daily. Thinking of switching to the patch. Do I need to wean off oxy before using the patch. Do i need to worry about od ing because of the halflife of oxy? So one day I guess they will tell me to stop taking oxy and start the patch i am assuming 100mcg to = what I am taking now. Any help would e great. Pros and Cons to the patch?
Avatar m tn I HAVE BEEN ON DURAGESIC FOR 6 YEARS, I WAS AT 100 4 MONTHS AGO THEN DRPPED IT TO 75 AND NOW AFTER 3-4 MONTHS I AM ON 50MCH PATCH... I CAN'T WAIT TO GET OFF THIS POISON.. ITS BEEN A 6 YEAR NIGHTMARE AND I AM DOING EVERYTHING I CAN TO MAKE IT END.. HAVE A HIP PROBLEM AND USED IT TO BUY MORE TIME UNTIL TOTAL REPLACEMENT.. THIS PATCH HAS BECOME MY WORSE DISABILITY.. ANYONE KNOW OF A DETOX OR WEAN OFF DRUG FOM HERE.. THE DROP ON THESE PATCHES ARE HORRIBLE..
Avatar m tn From what I've seen of fentanyl detox and my own year long, slow methadone recovery, I think fentanyl and methadone are only different in dosage. The full synthetic nature and side effects of both methadone and fentanyl make them very close tie for the worst detox ever. As I vomit and shat myself in my yard from methadone detox, I remember wishing I was a heroin or any other kind of addict. Anyway, mute point, but wanted to share.
