If you read the full prescribing info from Janssen on simeprevir, you will see that some variants or combinations of variants produce highly reduced susceptibility while Q80K by itself produces lower reductions in susceptibility. Since the COSMOS Cohort 2 patients who finished 24 wks of sof/sim all achieved SVR12, some despite Q80K, there is reason to believe that in that regimen, Q80K does not mean that a patient cannot take the combination.

I wonder whether simeprevir might not be a better choice for those with cirrhosis.
The success rates were high in the Cosmos trial with Sofosbuvir (Sovaldi) and Olysio (Simeprevir). However, these were not patients who have had liver transplants. Therefore, I do not know exactly how effective the treatment would be in people who have had liver transplants. I am not sure that it has been studied in liver transplant patients. Hector may know the answer to that question. He may also know what the success rate is. Hopefully he will see your question and respond.

"Early clearance is no predictor of SVR. That was true with old peg-interferon treatment based treatment with peg-interferon and ribavirin as well as when a PI was added. But, is NOT true for this treatment. In fact most people that treated with this treatment during the clinical trials were NOT undetectable by week 4 during the clinical trials.
To quote from the COSMOS study...
That's not possible unless the person was in the New York Cosmos trials. The meds everyone is on or coming off now have only been active since late Nov 2013, with S&O only being used since late Dec and for most places, since mid Jan of this year. So count out 12 weeks and then add 12 weeks and you are looking at May through to now as being the oldest responders to S&O. I finished June 27, so my three month free test is not until Sept 27.
