My doctor prescribed 3% lidocaine/0.5% hydrocortisone cream (which took two days to get). I was also using the hemorrhoid cream. In addition, I was using about 4-5 baby wipes to clean the area each time so that there was not one speck of anythiong left to irritate the hemorrhoids. Interestingly enough, about 6 days after that pain started, it began to decrease and after a couple more days, it was almost gone. I still had the external hemorrhoids but the hemorrhoid cream took care of them.

Ask the dermatologist how sure he or she is about the diagnosis, and why the LidaMantle for a fungus. I tried the blue sunburn gel with lidocaine in it as it cools and numbs the itching for a short while and after scratching it was painful too. Otherwise I also was given hydrocortisone 2.5% and then betamethasone 0.1% (both are prescription strength) I also found Benadryl helped alot but the tablets do make you drowsy.
For the external excoriation I used a xylocaine with hydrocortisone (prescription) cream. However, when it got more excoriated, I switched to Desitin, which was qite soothng and healing. It has been extremely painful and I have had difficulty even passing stools before using lidocaine. Doctor gave me lidocaine and hydrocortisone cream. It has helped a little bit, but my hemorrhoid does not seem to get better bc I have so many bowel movements each day, up to 5 or 6 per day. I'd imagine you've tried antihistamines for the itching, and possibly hydrocortisone ointment...How about a lidocaine (used for numbing sunburns and premature ejaculation) ointment? About three weeks ago, I posted that I had these terrible mouth sores. It was causing me so much pain that I couldn't eat or even open my mouth. My tongue was covered with blisters, my cheeks and gums were so sore and my mouth had cracks at both corners. My doctor sent an oral anti-fungal rinse and also something called "Magic Mouthwash" which has Lidocaine in it. It numbs your mouth so you can eat (and talk). The oral rinses cleared up the inside of my mouth in TWO days!

H cream and the Lidocaine cream. My plan now is to nip the first soft stool in the bud by taking Loperimide and hopefully I will ward off several days of passing shards of glass. I have to say, I could deal with the hemorrhoids and some discomfort or itching, but the Vertex company vastly understates the rectal "discomfort." It is not "discomfort," it is excrutiating internal rectal pain.
