Avatar n tn I had always averaged 6.5 prior to my Lantus issues, but had shot up to 9.3 or so recently. (High for me) Today, at-home testing showed a drop to 8.2, so I think I'm on a good track to continue the AM Lantus injections. Im also increasing my Humalog to equal 1 unit for every 10 g. carbs vs 15. Depends on what activities I'm doing. Also, I am taking a sliding scale 1-4 additional Humalog units in a.m. to cover high fasting bg...in addition to covering breakfast carbs.
Avatar f tn I am a type 1A diabetic with normal insulin sensitivity and predictable bg responses to insulin doses. Until now. I went on a reducing diet (high protein, next to 0 carbs, increased exercise) but never decreased my once daily PM dose of Lantus (10 units.) Immediately my AM bg's jumped to over 200 and stayed there. No hypoglycemia during sleep (I never woke up, anyway.)I could bring my daily bg's down with my shortacting insulin.
Avatar f tn One protocol is to take your blood glucose at night, then administer enough Novalog to bring it to normal levels. Then inject Lantus sufficient to bring the levels to normal in the morning.

Yesterday was told by a nurse that the Lantus I take is really low compared to amount of Humalog I take, but she never told me HOW much she thought I should be taking. I take 1.5 u Humalog per carb for all meals (for last 3 weeks - has changed many times). I'm supposed to take 11 u Lantus in am. My morning BS is usually 300-500.
Avatar n tn s truly scary (particularly when driving), and it took a lot of carbs to counteract. It seemed as though the entire Lantus dose took effect immediately (like a fast-acting insulin). After the third incident, I began to split my Lantus dose, taking half in the morning and half in the evening. I've had no problems since - but if I do, I know that I'll be dealing with the effects of only half as much insulin.
Avatar n tn I believe that the switch from evening Lantus to moring Lantus is generally made in the following manner but you should REALLY see a Dr. to do it correctly. Generally I think they change it over a long period of time by taking 1 less unit at night and then taking 1 unit in the morning and the following day take 1 less unit at night and 2 in the morning etc. I have never done the switch so I am not the best person to advise on it but that is what I understand.
Avatar n tn Then I did the same procedure again, removing another unit from my morning dose of Lantus and adding it to my evening dose. And wait several days to see what happens in the mornings. You eventually get to the perfect dose for you by doing it one day at a time with a few "let's see how this works" days in between to observe the morning sugars.
Avatar n tn Some of us prefer to split the Lantus dose, taking some in the morning and some in the evening to equal the complete dose. That way, only half is petering out after 20 hours, and the sugar levels stay more even throughout the day. The Lantus shouldn't work quickly, for it crystallizes in the injection site and slowly dumps tiny bits of insulin into the body all day. So the quick-acting insulin is what you want to use to lower a high reading.
Avatar n tn I went to a diabetes education class that teaches about the sliding scale and how to take your Humalog. However, my doc gave me a formula to use to counteract high blood sugars when not enough insulin has been taken. It's (bloodsugar) - 100 divided by 40. So, if your blood sugar is 300 you would subtract 100 to get 200. Divide that by 40 to get 5 units of Humalog. I also have a scale for when I eat. It's for every 15 grams of carbs I take 2 units of Humalog.
Avatar n tn Hi, If this low dose of lantus twice daily is working for you without any problems, there is no reason to change it. Lantus is written as once daily but many doctors(including myself) and patients split the dose because even though it is supposed to be "peakless", it is not for some people. At low doses it works especially well as a split dose. Levemir can be used instead, and is more commonly written as twice daily.
1353681 tn?1387083733 My insurance company pushed me to make that exact switch a few years ago, and I'm pretty happy with it. Biggest difference is that Levimir release cycles last around half as long as Lantus, which makes for 2 basal shots a day, and it's a little easier to accidentally overlap doses. For example, I try to get my shots at 7am and 10pm, but that creates a 3-hour overlap and gap on either end.
Avatar n tn Most patients require only about 80% of their old NPH dose when they switch to Lantus, so the fact that you are having to INCREASE your insulin dosage seems to indicate that the root problem may be insulin resistance. The fact that you write that your weight is normal and you eat a low-carb diet seems to make this the only logical explanation for the constantly increasing glucose readings. You may want to do some reading on this problem and current treatments for it.
Avatar f tn I've just been switched from Novilin NPH to Lantus. I'm to start at 10 units. for one to one correlation that seems to little, I was using 16 - 22 units at bedtime. Also is anyone combining both Novilin NPH and Lantus with Lantus at bedtime and the NPH at a lower dose around noon.

Avatar f tn My 23-year-old nephew who is 6'2" and weights well over 230 pounds only takes 34 units of lantus at night and up to 16 units of humalog if he eats pizza. I have told him that the lantus dose is governed by weight and since I weigh 220 and take 33 units a day, he isn't taking nearly enough. I think he is compensating with the humalog, which runs a higher risk that he will and does have insulin reactions. I also started out taking lantus at night.
Avatar n tn Keeping good records showed me that I got 18-20 hours of coverage At Most from the Lantus, and when I hit that blank spot between 4 and 8 AM, there was just nothing left. I ended up splitting the Lantus to a 2/3 dose in the AM, and 1/3 in the evening. That covered the 'no Lantus left' spot that was causing the mystery highs.
Avatar n tn I really think that this is a decision that all who want good control need to make for themselves. There are advantages to pumps and advantages to Lantus insulin as a basal dose. In my case, like you, I like the flexibility of the multiple shot routine and the shots don't bother me at all. Of course, flexibility is there with a pump, too, and the possible change in basal dose is a good thing perhaps with an exercise program.

Avatar n tn No, I am not aware of any people who have contacted us with any kind of resistance to Lantus. I do know that Lantus must be replaced at the end of a month on one bottle. The expiration date is really important, for near the end of the month, some of us find that it no longer crystallizes properly. Some people notice that when the Lantus bottle is about one month old, it doesn't work well. But if you just started the bottle, this is probably not the problem.
Avatar f tn That is absolutely impossible to say, especially for someone on an internet website! All diabetics are different. I'm assuming she is Type 2 as that is the forum you are posting to? It would depend on her degree of insulin resistance, her weight and other factors. Is she newly diagnosed? Didn't her doctor give her a starting dose? Is she on a basal/bolus regimen or just on the basal (Lantus). You say she is having trouble adjusting doses. What was she on before and what were the results?

Avatar m tn In addition to already being diabetic, I was disabled by a car accident/surgery that left me unable to do much of any reasonable exercise. I am on 1g twice a day of Metformin, which I understand is already a pretty high dose. I try to avoid high carb/high fat foods entirely, and rarely ever snack.
