Well it certainly appears that your diabetes and hbp are affecting your kidneys. Your GFR isnt horrible but you don't want it to continue dropping. Are you seeing a nephrologist? Make sure you see a kidney dr and that he knows all of the meds youre taking-even over the counter. Avoid any ibuprofen or any anti inflammatories as these are hard on the kidneys. You really need to make sure your diabetes and hbp are under control also. Your creatine is slightly high but not horrible.
I do think that continued high blood pressure can cause kidney problems. You've had this going on for quite awhile now so it's time to do something about it.
Most Drs will do nothing if ***'s below 139/89, because it's pre hypertension and they want you to try and control with it with less stress, diet and exercise. But 160/100 is hypertension.

In my opinion, investigations like kidney functions, blood sugar levels, ultrasound lower abdomen ,intravenous pyelography , thyroid function tests and cystoscopy may be needed to confirm a diagnosis. I sincerely advice you to consult a urologist or nephrologist and get yourself evaluated so that a confirmatory diagnosis can be made.
