It now makes perfect sense to me why you are in so much pain on the Fentanyl. Converting 360 mg of Oxycontin to Fentanyl results in 150 mcg/hr...this is conservative. Converting Fentanyl to equal 360 mg of Oxycontin with the opioid calculator results in 300 mcg/hr.
3112530 tn?1434032033 I was 150lbs when I started taking Fentanyl for pain issue. Fentanyl caused me to go down to 108lbs and have stopped the Fentanyl almost a year now. Currently 126 but want to go back to my previous weight. Was taking ensure but have lactose tolerant problems. How do I gain back the weight? Trying to eat a lot more pasta that I love but noting happening. Stuck at 126lbs for almost 6 months now. Any advise?
Avatar f tn ve been in oxycodone 10mg 4 a day and OxyContin 20mg 2 a day and due to other issues my pain Doc prescribed me the Fentanyl psych 25mg or mcg and took out OxyContin. Now two things.....1)my OxyContin was also used to treat tremors and restless arm syndrome, now that I'm off it, my restless issues and tremors are back and I'm trying to find out if I can do my OxyContin instead of oxycodone with the patch, but I would only do 20mg .

My options, according to the NP at the pain clinic, are the Fentanyl Patch, oxycontin, MS Contin, or methadone. The one I'm most familiar with is the oxycontin. All of them make me nervous.
Avatar m tn YES I was able to switch and taper from 62 Fentanyl down to 60mg OxyContin. That is quite a bit less OxyContin than the Fentanyl was but it was not hard and I did it all at once in about two weeks. I tapered a tiny bit on the Fentanyl and then stopped it completely and started the OxyContin. I did just fine with little to no w/d symptoms.
Avatar m tn Hello: The answer is NO. Fentanyl is the strongest of the opiates and you would be taking a huge step down. I tried replacing Fentanyl with Oxycontin and went into W/D's. I spent the weekend like that as the switch happened on a Friday. Monday morning I was back on the Fentanyl. The Doctor had suspected that this may happen. To give to insight, I was on a 75mcg patch and replaced it with 40mg of Oxycontin every 6 hrs. That was not nearly enough. I would suggest that you not do that.
Avatar f tn I prefer the fentanyl patch because it is all pain med -- fentanyl is a pure opiate agonist, like morphine, where as Butrans depends on a medication that is part agonist / part partial agonist. But either patch should be worn only by those who are opiate tolerant. Even the circular provided with the Duragesic patch warns against using the patch in those who aren't used to opiate analgesics. Have you tried Oxycontin?
Avatar f tn This just does not make sense that u had to lie that u were an addict and then only have options like sub. Be careful of the fentanyl patch even though it is excellent for pain relief. Fentanyl is 100 times stronger than morphine and 50 times stronger than heroin. Do the research. If you ever had to w/d from fent, suboxone for sure would not cover that. I tried the patch and let me tell you, I had noooo pain at all. I got off it just as quick when I did more research.
Avatar m tn Hello Novermhead, You've asked a good question. Oxycontin is a sustained release medication and I am surprised that you are taking four, 40mgs tablets per day. According to the conversion charts that I normally refer to the fentanyl patch would need to be 125 mcg to equal what you are taking now, which would be delivered over a 72 hour period.
535089 tn?1400673519 Sandee, I was floored too when My Doc said that 20mg of Oxycontin was equivalent to the Fentanyl. I was shocked and tried to explain this to him but at the same time, didn't want to sound like I was just wanting a higher dose. I was going to call a Pain Specialist in our City that deals with nothing but pain. My Mother used her when she had back surgery. I thought about a consultation just to see if my Doctor was wrong or there were alternatives.

Avatar n tn I went down to the 50 mcg patch and then went from the patch to oxycontin with percocet for breakthrough pain. Eventually, I got off of the oxycontin and just tapered the percocet down until I got down to 7.5 mg and then went off. I think you can just substitute the oxycontin and then gradually go down from there. I was on the meds for 12 years and went off last December.
1264863 tn?1391118193 You may feel some withdrawals from the Fentanyl, but with the oxycontin, it shouldn't be too bad. Once the fentantyl is completely out of your system the oxy will work better, just give yourself some time to feel the relief, it may take a bit. Good luck to you!
Avatar m tn i had to come off years ago from 72 mg a day on extended release ( taken off market forget name ) plus 4 80 mg oxycontin. i stopped the dilaudid with no problem and then weaned down on oxycontin which was tough. im on higher dose now and in conversion it says i need like 40 80 mg oxycontin to substitute?? i tried oral dilaudid but it gets me to nauseas. im worried if i use oxycontin ill end up on both. also is mscontin better to sub. for withdrawl than oxycontin.

135691 tn?1271097123 While I was in the hospital these past couple of days, they started me on Oxycontin 40mg sustained release tablet. I've previously been taking percocets and I'm finding that, the longer I take them, the less of an effect they have on me, which I understand is totally normal. At the hospital, they were giving me the 40mg tablet every 6 hours and I literally felt in a constant daze - which was fine, when I was confined to a hospital bed!

Avatar m tn Fentanyl is a stonger form of an opioid. Opiate and opioid mean the same thing: Opiate is an adjective while opioid is a noun. Percocet is an opioid as is fentanyl. They are synthetic which means they are man made and not derived from a plant. Good job on getting off the Percocet!

I was having severe constipation issues that were ridiculous!! I was experiencing severe bloating, cramps, chest pain, and difficulty breathing. I tried fiber supplements, stool softners, miralax, gasx, milk of magnesia and none of them worked.

Avatar n tn Is this something you have requested or is he/she concerned about other issues. It was my understanding that most physicians would rather prescribe Fentanyl over Oxycontin but I do know they all have their own personally opinion. And yes I would guess you would have some small withdrawal symptoms but the majority of them should be controlled by the new opiates you will be taking. 5410312 tn?1367531622 I was on Fentanyl 50 mcg for almost one full month. I just could not wake up in the morning. Once i did, my adrenaline got me going, but it was a night of 10-12 hours of sleep. I am a juv. diabetic for 40 years. I have adhesive capsulitis. Diagnosed only after unsuccessful shoulder surgeries diagnosed as torn cuffs. I had been on norco for 3 years, and then percocet for the last 2 years. The constant IR made me feel like trying an ER product. My selerium was changed from patch to 20mg.
