Fentanyl is a horrible drug and is 80 times stonger that morphine. I did a conversion once when I found a table online and was shocked to find out that the 100mcg patches I was on are equivalent to over 200 mgs of oxycodone. That's some bad stuff to get off of and there is no easy way that I have found. I had the cheap mylan patches (flat and not jell filled) so I ended up cutting them so I wouldn't get the full dose and tried tapering a bit.

400mcg/hr is the equivalent of 960mg of oral morphine!!! (a 50mcg patch is equivalent to 120mg of morphine). That is truly an astronomical dose!!! Tapering from fentanyl patches is difficult as they cant easily but reduced in small increments. I woukd suggest trying to taper slowly to about 100mcg (they make a 12mcg patch for tapering purposes) then switch to an oral medication and taper from that.

im not sure what mg but i expect him to start me on 15 of morphine. will the morphine help me? should i talk to him about another option such as percocet? im scared that the low beginning dosage will affect my quality and comfort even more.

So the 300 mg of codeine that you take is approximately equivalent to 45-90 mg of morphine. So your total morphine intake per day is 105-150 mg. The 25 mcg/hr patch is equivalent to the morphine dose range of 60-134 mg and the 50 mcg/hr patch is equivalent to the morphine dose range of 135-224. So either the 25 or 50 mcg/hr patch would give you dosing within the range of the morphine you are taking orally right now.
I was on Methadone prior to the Fentanyl. Fentanyl is VERY strong, they say almost 80 x stronger than morphine and the Dr will normally start you on a lower dose to see how it is tolerated. It does NOT work well for everyone and I didn;t really start to feel any pain relief at all until I was on the 75mcg patches. I also wonder what the conversion dose of Fentanyl is for the 200mg of OC you were on prior to the patch?
So you should be able to stop the patch and take a vic as needed. Duragesic (Fentanyl) withdrawl was the most painfull withdrawl I ever had. At one point I had a 160mg oxycodone habbit and it wasn't nearly as painfull as a duragesic 25 withdrawl (and I had only been on it for 30 days). My doctor messed up my appointment, leaving me 2 days from the time my 30 day supply was up until the next appointment.
It is so painful for them and dangerous even when they give them small amounts of Morphine to wean down. Fentanyl is no joke! I was told by my old uriologist that this patch would help me take less Vicoden. I had no idea what it was ti'll I decided to detox off it. Then I found out what it's like to want to die and beg God to just take you! I have detoxed off Vicodin HP, Norco 10's, etc....those were a freaking walk in the park compared to Fentanyl. But as Jaybay said, it is dispensed in micrograms to take care of the problem of getting too much at once, so try to comfort yourself with that and think of how much more convenient it will be for you to not have to take pills all the time and have basically a "built-in" pain reliever working for you. If you REALLY don't like how it makes you feel, take it off.

315-404mgs of morphine is equivilant to about 100 mcg of fentanyl. Thats oral morphine. The IV morphine chart is different. I do know that fentany is one of the strongest opiate medications available.. I believe it is much stronger than oral morphine. At least that is what I have heard and read. I also saw a show on TLC (Dr. G Medical Examiner) and she also stated fentanyl is the strongest opiate medication you can be prescribed. Be very careful with the fentanyl, do not abuse it.
My mother who has cronic back and shoulder pain is being taken off of fentanyl 100mcg patch and 7.5/500mg and being put on morphine 60mg 2x daily and then morphine sulfate 15mg every 4 hours for breakthough pain. What I'm wondering is there going to be withdrawl or is the morphine going to have enough of the same stuff in it. If not what kind of withdrawl systems will there be to watch for.
It will be VERY difficult to get you pain relief; you are used to the equivalent of a couple hundred mg of oxycodone-- meaning that you would get no relief from taking 40 of the standard percocet tablets all at once! ANY med will be a problem-- the amount of morphine needed would be huge, and would cause histamine release that would be dangerous. The only real option is fentanyl or sufenta-- both given by anesthesiologists.
