It turns out that portal vein and superior mesenteric vein are thrombosed. They put her on enoxaparin and wanted her to rest for 5 days until the next control. She refused to be put on heparin or warfarin because of her history of gastrointestinal bleeding and recent surgery. We've been told thrombectomy would be extremely risky, given her medical history.
I read somewhere enoxaparin only prevents further occlusion of the vein and does not lyse existing clot.

continue through with warfarin, or to stop the warfarin and bridge in between with enoxaparin or heparin (as snconorm had done).

So continuing through with warfarin seems to be the newer strategy.
What would NOT be done is to stop anticoagulation altogether.
Yes I'm on the generic enoxaparin. I was on it with my first and forth (all 4 pregnancies were miscarriages). Now I'm on it again since 3 1/2 weeks pregnant.
This will explain vector adjustment. Then the NAPA nomogram is used for the LRI on the vector adjusted Axis. NAPA (Nichamin, Age Pachymeter, Adjusted).

Hi, Thanks for all of this! I am so glad I caught you on line. I do not see that he has that test ordered for me. I am going through my files here because I have had a ton of blood tests but I don't see anythinglike that. I have some chart (nomogram?) for a PTH test, thats all I see related to thethyroid. I think my doctor was a little caught off guard when he read the test results. He was upset that no one had called me and that I was unaware of this when I entered his office.
The doctor simply follows a standard nomogram based on the prism diopters of esotropia. If you are having a bimedial recession, usually the same millimeters of recession is done on both eyes. Your are trying to bring the eyes together into fusion. If surgery successful, double vison can be fixed right away or a day or so. There is a 10-20 chance or so of needing second surgery (ask your surgeon for exact numbers.

The answer you got depends on the nomogram used. You used the DONO. If you had used the NAPA (Nichamin Age Pachymetry Adjusted); your eye will need two 70 degree arcs (I put in an age of 65). However, this is depth adjusted and a peripheral pachymetry needs to be done. Most doctors are not comfortable correcting this much astigmatism and do not do depth adjusted LRIs. This require a micrometer adjusted LRI blade which is much more expensive that a fixed depth blade.

I will pm you the link to the chart, but you are well below the values - so your doctor is likely to tell you that you do not have CAH.
Cushing's is very similar in testing and symptoms. You did go up a bit from the stim test but it was not done correctly IMHO as even for Cushing's the intervals are 30 and 60 minutes from baseline.

I have implanted about 50 Crystalens HDs.
1) Near is about 1.5 lines better. Most patients see J1 to J2. This is the distance eye (dominant). Most near eyes are J1 to J1+
2) Target is easier to hit. Of the 31 stable patients 85% are within + -.25 of target. The two worse eyes are +.75 and -.75 from target. Not so with AT50 and AT45.
3) I target the dominant eye for plano and the nondominant eye for -.50. Micro monovision.

Once I was there I got an ultra sound and it was was confirmed I had a DVT on the left side of my groin. I was sent home with enoxaparin injections and stockings and another appointment was made in a better hospital (yesterday) for further assessment.
Unfortunately I was told there was not a lot they could do for me as the meds that break the cloths down are way too dangerous for me to take as I will have cloths all over my damaged organs just sitting there doing their job.
in 1998 had a bypass that did not work they put a defribilator in 2000 which has worked well up to august this year i was rushed in with a heartbeat of 161 they put me on amiodrone straight in the arm for three days my defib went off twice but did not bring me back into rythem they rushed me to Barts in London where they took the drip of amioderone straight out of my arm because it caused flebitis and put me on oral amioderone took me off warfarin and gave me enoxaparin injection till the warfar
I suspect that my hot, red forehead; flaky facial skin spots; ringing ears; & hot torso & head with no fever whatsoever..... may be due to taking WARFARIN too long.... also known as COUMADEN.... also used as RATPOISON in high doses... Doctors like to put you on Warfarin for 6 to 12 months at a time if you have had some major trauma, just in case you may develop a blood clot.
