Avatar m tn Didn't know where else to put this, but I was recently switched from Morphine 60mg ER to the Fentanyl Duragesic patch. I thought the weird flu like symptoms might be a result of the new drug, but apparently it has to be withdrawal from the Morphine. I'm kind of miffed neither the doctor nor pharmacist even mentioned this could happen. I was naive and thought the new medication wouldn't create an issue with the switch, I was wrong.
Avatar n tn There is no exact conversion ratio among opiate pain meds that fits everyone. First and obvious is to go back to what worked. Then, if you have a real need to change to fent, titrate slowly down w oxycodone and up w fent. Second is to understand why the decision to switch. Fentanyl patches are great for some people. Not for everyone. Insurance is another question. They're just being the cold blooded, money grubbing scum both insurance and Pharma always are while bulking you out of $$$.

Avatar f tn I am on the 50 mcg/hr patch and inside the prescription leaflet it shows morphine conversions to up to 300 mcg/hr of fentanyl, which is equal to three 100 mcg/hr patches. Because you are on 100 mcg/hr of fentanyl, keep in mind that the other "newer" medications that weathergirl mentioned looking into, will require a large dose to equal the amount of fentanyl you are getting through your current dose.
Avatar n tn All I know is that they will use a conversion chart to do that transition. I asked the PCP what to expect, and she said that I would feel some discomfort for the first couple of weeks but that it wouldn't be bad. She really wasn't convincing though. I have been on 75mcg/q48hrs for 4 years. I am sure that I am very dependent on this stuff. Why all the fear you may ask??
Avatar m tn )is dumbfounded as to what to give me for pain. I told him maybe a LOW dose quick acting MORPHINE that will get in there cut the bone pain and not hang around like this patch does.. He wants to give me junk like percosets, or loitabs or tylox, percodan, He even mentioned methadone.. I told him after coming her I had rather slit my wrist than take that and have to wean off it one day. I have to have something until I get a hip replacement.

Avatar m tn I'm sorry to hear of your friend's pain and illness. I don't believe (although I'm not 100% sure as I"m not a pharmacist) that the fentanyl patches contain any morphine. However, if your friend is concerned or worried that since he's had a bad reaction to morphine that he will also have a bad reaction to the fentanyl, I suggest he ask his pharmacist.
Avatar f tn I used fentanyl patches for a year and had an extremely high tolerance for opiates, 1 100mcg/hr has about 10 mgs of fentanyl. Fentanyl is 40 times stronger than morphine. So in essence, chewing one 50 mcg/hr patch is about 5 mgs of fentanyl which is equivelant to 200 mgs of morphine. That's like doing 2 and a half 80s at once. What they're doing is dangerous, end of story, take it from some one that knows first hand. I'm not a dr.
Avatar f tn Gracie, I need to make clear to you that the Oxycodone is made in both the Long-acting and the short-acting med. The Percocet that i take is the Short-acting of course. I was assuming that you were talking about the short-acting. If you are talking about the Long-acting, it's still my understanding that the Fentanyl is still the stronger of the 2 meds. I'm sure that others will be along that might have more information. for you.

Avatar n tn I had a long converstion with the pallative care physician and he told me that every precaution will be made to keep me out of a relapse. I am going to have to have a major surgery to remove my pancreas in a couple of weeks. I will be in the hospital for 10 days. I am being told that this will fix the problem without chemo or radiation. I have a good friend who passed away Wed from the same thing. I guess I would just like a little feed back on the best thing to do.

By the end of the week I was sick to my stomach but I think that was from the abrupt discontinuation of the patch.. Avinza killed pain better than /Duragesic but I was scared of it because dr said it was MORPHINE.. slow release.. Is it really that easy to come off of should I need it ?? did you use it for 6weeks and just stop with no w/d??
Avatar f tn I do trust my doctor and he is a very reputable physician that has been featured on the local news in our state but despite this, he did not apply a cross tolerance at all when he switched me from morphine ER to opana ER (you may remember responding to my post on this subject awhile back). In fact, it was even more medicine than I was already taking in both morphine ER and breakthrough pain medicine in the two 30 mg opana ER pills I take per day.

By the time I got the Rx filled an the patch on me ( that was about 4 hours ago ) most of the nice releive i got from the morphine was gone. I am now getting the creepy crawlers much like withdrawl and am very very cranky. My question is: Is 12mcg/hr of this stuff very much. The info I can get tells me its about 1/10 of a 500 vicodin an hour.

Dr. wanted me to drop 30 mg morphine every 2 weeks which was too fast so I thought going to subutex would help get me off high dose. 2 weeks ago I detoxed and have not stabilized. Started at 6 mg. now at 4 mg. Also wonder if traded bad for worse habit. I don't feel good on this med. Go between WD symptoms to fogginess and dizziness.
890735 tn?1261421725 Hi, I am not sure of the exact conversion rate but Fentanyl is 40 times stronger than morphine so taking that into consideration it sounds correct. Also doctors normally start Pt's out on the 25mc of Fentanyl and then titrated up as needed. Rarely is a Pt started out on higher doses. This way your put on the lowest amount that will treat your pain. Tolerance issues being considered the lowest dose is the best. I hope this answers your question.

So far, it seems to me to have been about equally effective, possibly a bit more effective, than the morphine. The only reservation I have is that the effectiveness doesn't seem to quite last all the way through the 12 hours between the times that I take the pill. Any thoughts? Thanks!
