Avatar n tn My doctor reduced me to the above dose and its going terribly. The conversion for the fent for that dose is 50 to 75Mcg. My dr said to try changing the patch every 2 instead of every 3. My insurance won’t pay for this non-standard dose. Idk what to do. I’m in constant pain, sweating crazy - i’m in a nightmare.
Avatar n tn All I know is that they will use a conversion chart to do that transition. I asked the PCP what to expect, and she said that I would feel some discomfort for the first couple of weeks but that it wouldn't be bad. She really wasn't convincing though. I have been on 75mcg/q48hrs for 4 years. I am sure that I am very dependent on this stuff. Why all the fear you may ask??
Avatar m tn t know where else to put this, but I was recently switched from Morphine 60mg ER to the Fentanyl Duragesic patch. I thought the weird flu like symptoms might be a result of the new drug, but apparently it has to be withdrawal from the Morphine. I'm kind of miffed neither the doctor nor pharmacist even mentioned this could happen. I was naive and thought the new medication wouldn't create an issue with the switch, I was wrong.

I once walked in a room as a nurse, found a patient who had taken off his patch and chewed it...he coded and died...there is alot of medication concentrated in the patch to be released over a 24 hour period...not all at once...if you cut them you would have no way of knowing the amount your wife would be absorbing..

He wants to give me junk like percosets, or loitabs or tylox, percodan, He even mentioned methadone.. I told him after coming her I had rather slit my wrist than take that and have to wean off it one day. I have to have something until I get a hip replacement.
Avatar f tn medications that weathergirl mentioned looking into, will require a large dose to equal the amount of fentanyl you are getting through your current dose. Of course, there is cross tolerance but once you get that far up in strength via fentanyl, you are still more than likely going to require larger doses than normal of morphine, oxymorphone, oxycontin, etc. There is also an option to ADD another long acting medication in with the fentanyl.
Avatar m tn For me, the problem with transdermal fentanyl was a rapid development of opioid tolerance. Within 18 months of use, my dose had gone from 25 to 100 ug, and I needed another adjustment upward. Instead of doing that, my doc and I chose to use another long-acting opioid medication. Avatar m tn Something else could be affecting the change of biking velocity, or something could be affecting the effectiveness of the dose but possibly it does not have that much of an impact on the biking. In other words, do talk to her doctor about her dose, but check your assumptions about cause and effect before suggesting a change.
Avatar f tn This rotation calls for careful titration or the new dose and can require multiple visits to find that dose. The last time I rotated from oxycodone to morphine, it took about a month to titrate to the proper dose, which meant I was in the doctor's office about once a week during that month. Undertreated pain is misery. Be persistent in your goal of properly tirating your new, effective dose. Keep a pain diary, and stay in close touch with your doctor's office.
Avatar n tn I am 11months and 5 days clean and sober and am enjoying life being sober!!!! I was given dilaudid in the hospital and I have been sent home on the Duragesic Patch. I had full disclosure with the doctor and he knows my entire history. Unfortunately, the pain is to bad to just be controlled with over the counter meds and I was told that Tylenol will just aggrivate the situation. Needless to say, I am scared to death.
Avatar n tn I have been on the Duragesic Patch 74 mcg for about 3 1/2 years. During the last 6 months, I have become extremely depressed. I notice that it is worse after the second day any given patch is on. I have been treated for the depression with different antidepressants and so far have not found one that works. I am so desperate for help as the depression has taken over my life. Has anyone out there experienced this? Any input would be appreciated. I feel as though I am running out of time.

ITS BEEN A 6 YEAR NIGHTMARE AND I AM DOING EVERYTHING I CAN TO MAKE IT END.. HAVE A HIP PROBLEM AND USED IT TO BUY MORE TIME UNTIL TOTAL REPLACEMENT.. THIS PATCH HAS BECOME MY WORSE DISABILITY.. ANYONE KNOW OF A DETOX OR WEAN OFF DRUG FOM HERE.. THE DROP ON THESE PATCHES ARE HORRIBLE..
Avatar f tn s temp protocol calls for timed released T3 doses that are increased by 15mcg per day up to a set dose and then back down by 15mcg per day until the dose reaches zero. This is supposed to reset my temp and hopefully make me feel much better. The cycling isn't supposed to continue for more than a couple of months after which the body temp should remain normal. Hopefully. I am a 62 year old retired teacher. I have struggled with migraines since I was 9 years old, depression since I was 14.

Withdrawal: After the patch is removed, it takes 17 hours or more for a 50 percent decrease in serum fentanyl concentrations. If you are planning to discontinue use of Duragesic, a gradual dosage decrease is recommended since it is not known what dose level the drug may be discontinued without producing the signs and symptoms of abrupt withdrawal.
640719 tn?1277140030 So goolarra is correct that your new dose is low. Just be aware of how much of a reduction the new dose represents and the likelihood of degradation in your symptoms. Which leads me to the question: what is the cause for the change in meds? If it was because of you still having hypo symptoms, then I see little risk in switching to a full grain of Armour, but do it in two stages. That way you should have the best of both possible outcomes.

Her case seems to be unique for a number of reason that I can expand on if anyone out there needs more information. Our child has been doing very well once we finally found the medication Lexapro. She was a year into taking 30mg daily plus Clonepin (.25mg 2x daily) and doing very well. Recently, she started going down hill.

445808 tn?1206876442 i think it is a normal side effect of methadone regardless of dose, i was on a low dose for almost a year 20-30 mgs, and i sweated like mad every single day, nonstop! it was awful and very embarrassing...there wasnt 1 day in that year that i didnt sweat...it was the same with suboxone at first, but that did eventually stop....thank goodness!!
