I tried a dandruff shampoo that contains zinc pyrithione, but this time I rubbed the shampoo directly on the spots instead of just the foam. My ears and groin are still red but the scales and scabs are virtually gone in less than 10 days and I only itch when the skin is wet (sweat or post-shower).

I've had the same thing for a long time. I've tried everything. I happened to be reading about pyrithione zinc ...which is the active ingredient in dandruff shampoo. Turns out it's also used for dermatitis, etc....I tried it.....it works. Could see a difference after 2 showers. Is now almost gone completely.

Tinea capitis is a fungal infection which can be responsible for dandruff and hair fall also. Treatment includes washing the scalp with shampoo with selenium sulfide (2.5%) or zinc pyrithione (1-2%) used 2-3 times weekly at home. I picked up some Neutrogena T/Sal last night and used it before bed and this morning. Seems to be working great. I think I might switch back and forth between this and the head and shoulders, unless the T/Sal continues to work this good on its own.
Both have the active ingredient Pyrithione Zinc in them which is the same ingredient they use for dandruff too. They do add eucalyptus as well in the itchy scalp version, which seems to relieve scalp issues more when added to that Zinc stuff.
One other option is Selsun Blue's "itchy dry scalp" formula which besides being a Extra hydrating formula, contains 3 percent salicylic acid, which normally is added to face washes because it works great keeping zits at bay.
You need to use medicated shampoos containing coal tar, ciclopiroxolamine ketoconazole, selenium sulfide, or zinc pyrithione. For the acne, you can apply Benzoyl peroxide 4% on the lesions.

The best way to treat dandruff is to use a dandruff shampoo. Look for shampoos with ingredients such as zinc pyrithone, salicyclic acid, coal tar, selenium silfide, or ketoconazole. All of these ingredients have been shown to work on dandruff; to use the shampoo, apply it to your scalp and hair, rub vigorously, and allow it to sit for five minutes so that the ingredients can work before rinsing it out.
After finishing the Clobex treatment (which did not help) I went off all prescription shampoos and started using all natural Neem shampoo which got rid of the dandruff. But the hair continued to fall.
I went back to my regular doctor to get hormones, thyroid, allergies, vitamins and everything else tested and it all came back fine, except a copper and zinc deficiency.
