After six years post MI and ef varying between 25-35%, my cardiologist now feels that I should shift from aspirin(150)+clopidogrel (75) to warfarin. Two years back he did tell me that he is double minded on whether to put me on warfarin. But this time he appears to be very sure and has insisted me to go for warfarin.
His justification is "that is what literature says".
I have no AF or valve problem.
I suffered a mild stroke (blood clot) Consultant prescribed Clopidogrel but cannot tolerate side effects. Then consultant prescribed Dipyridamole. I cannot tolerate this either. Severe headache, vomiting and diarrhoea. Has anybody got a different medication they are on?

Still I did not want to take a chance and therefore I started on very low dose- 5 drops twice daily and increased it to normal dose of 15 drops twice daily over a period of 15 days. I take it mixed with 3-4 teaspoons of water once morning and once in the night keeping a gap of 30 miniutes before and after for taking any eatables.
Now I am taking it for over 40 days and I feel better.

The only info I have read about the dose change so far has been tied to Vertex gradually declining quarterly performance and how they hope to improve the side effect profile and encourage more people to treat now rather than wait. I am guessing the OPTIMIZE trial shows twice daily dosing is equally efficaceous.

If this low dose of lantus twice daily is working for you without any problems, there is no reason to change it. Lantus is written as once daily but many doctors(including myself) and patients split the dose because even though it is supposed to be "peakless", it is not for some people. At low doses it works especially well as a split dose. Levemir can be used instead, and is more commonly written as twice daily.
I am currently taking plavix, 81mg aspirin AND coumadin on a daily basis. I had a stent put in in July 2008 and triple by pass in 2006. I'm 64 and had my first MI at 33. My father died of an MI at 46, my EF is under 30%. My EP (at one hospital) just suggested I eliminate the plavix while my CHF specialist (at a nearby major research university) said I would be on plavix for a "long time" (forever?).

Ativan is a mid-acting benzo compared to clonazepam which is long-acting. ativan's half-live is 14 hours, but clonazepam's is between 20-50 hours. thus, you are experiencing the symptoms you mention simply because it take a lot longer for clonzepam to start kicking in. are you taking 1.5mg clonazepam only once a day? klonopin is best when taken over a course of twice or three times daily.
as far as dosing, ativna's max dosing is 10mg daily and klonopin's is 4mg daily.

My problem is I am having monthly to twice monthly breakouts of hsv sores on the margin of my lip. I have taken 1000mg Valtrex daily for a few months now and I am still getting very frequent outbreaks. This is becoming mentally and physically taxing. Anxiety levels never get a chance to subside which creates a viscous cycle I am sure. Do you have any thoughts for me? Do I bump up to 2000mg Valtrex daily to get this under control? IS this normal? Is there any hope of this subsiding with time?
I was very recently prescribed Wellbutrin XL, after trying several medications that have not helped [including Wellbutrin SR]. When I saw my doctor, she simply said 'I'm putting you on Wellbutrin twice daily' and I assumed it would be the SR. But what I was given was XL, with the instructions to take it twice a day [morning and around noon]. My doctor's office is closed today, or I would have asked her. I did ask the pharmacist, and he said that yes, this is okay.
