Dear Dr Rockoff,
I'm looking for some advice/reassurance. I have been off accutane for a little over a year, during which time I have had at most one zit at any one time which would disappear relatively quickly. In the last few days, however, I have had two zits appear simultaneously, and I'm slightly worried. I've been using benzoyl peroxide religiously for the past several months, so now I'm questioning whether the positive effect of the accutane may be over.
After that i started getting a lot of pimples on my cheeks so the dermatologist put me on antibiotics for 3-4 months. My acne just got worse so i was put on accutane for 2 months and it cleared my skin. By that time i was 17 years old . After two months i started getting pimples again . So for a month and a half i had antibiotics again but they were completely not responsive. My acne got worse than ever before. I did not have systic acne but it was pretty bad .

Treatment for such acne like bumps includes topical and oral antibiotics, topical retinoids, hormonal therapy, dermabrasion , phototherapy and laser resurfacing.
Normally a combination of treatments is followed and if nothing proves effective then dermabrasion and laser therapy are useful. You can discuss these treatment options with your dermatologist. Till then follow the treatment that has been prescribed to you and use Accutane only under medical guidance.
Why do you wait a year to prescribe topical treatment after Accutane? What skin care regime do you recommend? Do you prescribe antibiotics if the Accutane doesn't perform well? It has been difficult to get rationale for Accutane treatment. Thank you for your advice...
A complete evaluation of your acne problem is necessary. Other options may be available for acne treatment. The severity and extent of your acne has to be considered here. There are certain factors to consider when using accutane. This includes pregnancy , use of other medications and psychiatric side effects

17 year old daughter having stomach issues, just got a positive test for celiac, has bad acne, derm wants to put her on accutane. I am worried about the accutane and her stomach. Not sure celiac is only cause of stomach aches. Also birth control pill used to control acne, then she went off and it came back worse than ever. Antibiotics and other topical have not worked well enough and the dermatologist is recommending that I go on accutane. I have an app. about it in several weeks. My father had severe acne until he took accutane for 3 months at a high dose without recieving any negative side effects, which cleared up 90 percent of the acne. My mother didnt have acne. I took roaccutane in my late teens and it cleared up my acne wonderfully for a few years, but then it came back, and worse. My skin is much more sensitive now than it ever was before - rashes all the time.. Also, there have been big 'hoo haas' about whether or not it causes depression. I have also suffered a number of clinical depressions since.
The best thing I've found for acne is Brevoxyl (4% benzoyl peroxide). It's a cream, available over the counter.
If long-term use of anti-biotics has proved ineffective, he may be a candidate for oral accutane. Discuss this with your doctor, and check out our acne forum too.
Hello. I have been on Accutane for the last 5 months. My acne is clearing and I have no new breakouts, however I have lots of red marks where the acne was (post inflamatory hyperpigmentation possibly?)? Anyway, I was just curious if blue light or red light treatment would help, and can I use the light treatment while on Accutane? If not, what else could I do to reduce the appearance of the red marks? The acne is on my chest and is very hard to look at.
s opinion on the effectiveness of Accutane on smaller acne heads. My acne is on my face, neck, back, and chest. There are not very many big acne heads (I have benzoyl peroxide5%, Differin 3%, and Minocyclin 100 mg twice a day). These medications play a big part in my not having many big zits.
Anyways, I have lots of little acne heads all over. How effective is Accutane at getting rid of these heads and keeping them off? (from your experience or from official sources)

There is no miracle cure for acne. Cortisone shots reduce inflammation and allergic symptoms but can actually induce acne.
The drug considered the "gold standard" treatment for acne is Accutane (oral Isotretinoin). This is a power drug for people with severe acne which has not responded to standard treatments.
Thank you for the replies. We have decided to hold off on the accutane for at least a year. My son is on yet another bunch of prescriptions (tazorac gel instead of the cream-as well as doxycycline 100 mgs 2x a day, prescription benzoyl peroxide/salicylic acid pads) and so far his skin is much better. After talking at length with my husband and our son, we decided that the chance of stunting his growth, as well as the joint/muscle pain would be too much of a risk.
I am not a doctor but I did work as a pharmacy tech for 4 years, and personally it scared the heck out of me all the warning labels we had to put on Accutane. If you only have mild acne I'm surprised your doctor would prescribe this medication unless it really bothers you and you have exhausted all other avenues. For a lot of people acne, especially just mild acne, is just part of life. Good luck to you whatever you choose!

Hi I had been on accutane for only 2 months. This means i did not complete the 6 month treatment. Right now i have been off accutane for 2 months and a half.Right now i have occasional pimples but nothing great like before. My face is getting back to its normal greasy self.I have many embarrassing scars on my face, My question is, is it okay to have laser treatment on my face or microdembrasion or do i suffer the possiblilty of having my acne scars worse than they are now?
I have had a problem with acne all my life and took a course of Accutane in my early twenties. I was acne free for 10 years, but then it came back and I developed tiny bumps under my eyes that my dermatologist said were syringoma (after a year of treating it as acne with retin-a and antibiotics). As usual, the antibiotics and retin-a did not do anything to treat my acne problems, so I have begun a new course of Accutane.
My daughter has been on Accutane for 2 months. While her acne is mild, we put her on this due to my hx of cystic acne at age 22 which was completely responsive to accutane. My question is , she has not seen much change . She is still breaking out, and states zits are larger than in past, but do reslve more quickly She was on accutane once in past with immediate response in first month. Dr just increased dosage. What can we expect in terms of response?
